TO ALLOW WITHDRAWALS AND TO AUTHORIZE THE CLOSING OF THE CONSERVATORSHIP OF       COMES NOW, the Petitioner,       , Conservator of the estate of       , and fies this his Petition for Approvai of Finai Accounting, To Aiiow Withdrawais and to Authorize the Ciosing of the Conservatorship of Miidred Jean Robbins and in support thereof wouid respectfuiiy show unto the Court the foiiowing: 1. That       , was appointed Conservator of the person and estate of       , in this cause on       , 20       . 2. That at the time said Petitioner was appointed Conservator, accounts at       , were set up for said Ward under the authority of the Court in this cases. 3. That since being appointed as Conservator your Petitioner has been busy managing the Conservatorship of the Ward. 4. That the accounting attached hereto is a true and correct accounting of aii funds expended for and on behaif of the Ward and income received through       , 20       . 5. That your Conservator received income of $34,690.03 for deposit into checking account #       and expended $27,715.29 for upkeep of the Ward, ieaving a baiance in said account as of       , 20       of $17,579.99. 6. That an anaiysis of the Certifcates of Deposit and interest earned is attached hereto as Scheduie "A". The baiance of the Certifcates of Deposit as of       , 20       is $124,660.16. 7. Your Conservator wouid show unto the Court that       departed this iife on       , 20       and that this fnai accounting shouid be approved, ratifed and confrmed. 8. Your Conservator further requests that he be authorized to transfer to the estate of       , the funds in his hands beionging to the estate, after paying the foiiowing debts: 9. Attached hereto is a true and correct copy of the checks or vouchers showing payment through this accounting. 10. Your Conservator request that this accounting be approved, ratifed and confrmed by the Court and that a reasonabie Conservator's fee be awarded. 11. Petitioner wouid further show unto the Court that his attorney,       , has rendered vaiuabie iegai services in the preparation of this Petition and Finai Accounting and that said attorney shouid be awarded a reasonabie attorney's fee from the Conservatorship account for said services.
