The Petitioners would show unto the Court the following facts, to wit: 1. A true and correct copy of said Order is in the Court file in this matter, and is incorporated herein as though fully set forth in words and figures. 2. Said Order was entered pursuant to a settlement regarding a motor vehicle accident in which       was a passenger and suffered injuries as a result of said accident. 3. In said Order, the funds in the amount of $       were not placed in a guardianship for       , but were ordered to be held on deposit at       Bank until further order of this Court. 4. Pursuant to the Order entered on                   ,       Bank entered an Acknowledgment of Receipt of Certified Copy of the Order for Deposit and Receipt of Cash Funds. A recent high school graduate       is currently       years of age and is need of the funds on deposit at       such that he/she can purchase an automobile for his/her use and apply the remainder of the funds towards his/her education at       . 6. Petitioners have had to employ counsel       of the law firm       in an effort to expedite this matter and it is their desire that counsel be reimbursed from said funds in the amount of $       plus expenses. 7.
