Waiver and Release from Liability for Pedicure Services at Trade Show Whereas, _____________________________________________ (Name of Company), hereinafter called Company, is offering free pedicures to the show attendants at _____________ ______________________________________________________________________________________________________________________ (Name of Trade Show, City and State); and Whereas, the undersigned attendant, hereinafter called Attendant, desires such a pedicure; and Whereas, it is a condition of ________________________________________________ (Name of Company) that no pedicures be given to the show attendants at said trade show unless a release in this form is signed by the attendant; Now, therefore, for and in consideration of the mutual covenants contained in this agreement, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree as follows: The undersigned Attendant does hereby waive and release, indemnify, hold harmless and forever discharge Company, its agents, employees, officers, directors, affiliates, successors, members, and assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to the pedicure services being provided to me by Company.By this Waiver, I assume any risk, and take full responsibility and waive any claims of personal injury associated with such pedicure services by Company. The provision of this Waiver and Release will continue in full force and effect even after the termination of the services being provided to me, whether by agreement, by operation of law, or otherwise. I acknowledge, represent and warrant that I am not diabetic nor do I have poor blood circulation. I further acknowledge, represent and warrant that I do not have a abnormal skin condition, and I am not pregnant. I have no sickness or condition that would prevent me from safely receiving the pedicure that will be performed. I authorize Company to record photographs of me, and to use such photographs in any manner of media whatsoever, including unrestricted use for purposes of publicity, advertising and sales promotion. I have read, understand and fully agree to the terms of this Waiver and Release. I understand and confirm that by signing this Waiver and Release, I have given up considerable future legal rights. I have signed this Agreement freely, voluntarily, under no duress or threat of duress. I am 18 years of age or older and mentally competent to enter into this Waiver and Release.
