Fill and Sign the Release of Liability for Student Travel off Campus for Field Trip Form

RELEASE In consideration of being allowed to participate in the _______ ______________ Section Meeting of the ______ in _______ , __________ , and with the understanding that I desire to provide my own transportation to and from said Meeting rather than traveling in the College’s vehicle, I hereby waive, release, and discharge _______ College from any and all claims for damages for personal injury, death, or property damage which I may have, or which may subsequently accrue to me, as a result of my participation in and travel to and from said Meeting. This release is intended to discharge in advance _______ College (and its respective agents and employees) and the ____ (including the _____________ Section) from and against any and all liability arising out of or connected in any way with my participation in this Meeting, including any liability arising out of my providing my own transportation to and from this Meeting. I further understand that serious accidents occasionally occur during automobile travel; nevertheless, I hereby agree to assume those risks and to release and hold harmless all of the persons or entities mentioned above who might otherwise be liable to me (or my heirs) for damages.
