(street address, city, county, state, zip code) Whereas, Releasee rents out a photographic and television production studio (the Studio ) located at       by the hour to (street address, city, county, state, zip code) various photographers and television producers; In further consideration of being permitted to use the Studio, Releasor, for himself, his legal representatives, agents and assigns, hereby releases, waives and discharges Releasee, his employees, agents, and/or assigns, all of whom are referred to hereinafter as the Releasees , from all liability, damages, or injuries to the Releasor (including death), or to anyone allowed in the Studio by Releasor. Releasor agrees to indemnify the Releasees and hold them harmless from any loss, liability, damage or cost any of them incur due to the use of said Studio or equipment by Releasor in any manner whether caused by the negligence of the Releasees or otherwise. Releasor hereby assumes full responsibility for the risk of bodily injury, death or property damage due to the use by Releasor of said Studio. Releasor expressly agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the state of       , and that if any portion thereof is held invalid, it is agreed that the balance shall continue in full legal force and effect. Releasor has been specifically warned by Releasee of the following situations and takes full responsibility for any injuries that result from these situations. 1. Releasor must properly use and weigh the stands when the lights or flashes are in use since if improperly used they may fall and cause injury. 2. The studio will not be supervised and no security will be provided. Clients will use a PIN code to enter the Studio. 3. An electric shock may come from the equipment of Releasor if Releasor does not Release, Waiver of Liability, etc. Page 1 of 2 correctly use the equipment in the Studio. 4. Clients will receive a tour of the studio from Releasor and an explanation of injuries that can result from misuse of the equipment. This tour and explanation must be given prior to the use of the Studio. 5. Prior to the use by the Clients of the Studio, Releasor’s Clients must agree that each is familiar with and is competent in using the equipment that will be used in the Studio. 6. Releasor will require that the parent(s) or guardian(s) of any minors in the Studio be supervised at all times and must never be allowed to touch any or go within       feet of any equipment.
