About the North Carolina Administrative Office of the Courts The mission of the North Carolina Administrative Office of the Courts is to provide s ervices to help North Carolina’s unified court system operate more efficiently and effectively, taking into account each courthouse’s diverse needs, caseloads, and available resources. Forms Report – Introduction | January 1, 2018 Page 1 Introduction This is the July-December 2017 NCAOC Forms Report, a document that lists change and posting dates for the various NCAOC forms during the time period beginning July 1, 2017 and ending December 31, 2017. The next (January -June 2018 ) installment of the Forms Report will account for changes to NCAOC forms during the time period beginning January 1, 2018 and ending June 30, 2018 . Each new installment of the Forms Report will be posted to the “Complete Index to Judicial Forms” link on the Judicial Forms Search Page ( www.nccourts.org/Forms/Documents/CompleteIndex.pdf ). Thi s document contains a listing of official NCAOC forms. In some cases, forms and documents that are not NCAOC documents (e.g., the adoption forms maintained by the North Carolina Department of Health and Human Services) are linked -to on the NCAOC forms page . In a similar manner, envelope templates and other documents in use by NCAOC that do not constitute judicial forms are also posted on the NCAOC forms page. This Forms Report does not include non -NCAOC forms and, with few exceptions, it does not include do cuments that are not given an “AOC -” number. Forms in this document are grouped in categories from the current NCAOC form numbering system, irrespective of general areas of practice that the forms may in fact relate to (e.g., civil revocation forms are grouped under the Civil Revocation Forms heading and not with Criminal Forms). The listings of “Forms Currently In Use” include forms that are in regular use, as well as forms that are used more infrequently. Where a form is currently posted to the Judicial Forms Search Page ( www.nccourts.org/Forms/FormSearch.asp ), a link to the Forms Search Page posting is included within this document. As many of the NCAOC form s are frequent ly update d and revised , use of a form’s presently -posted version is highly recommended.
