Sample Military Leave Policy Language It is the policy of this company to grant leave as necessary for uniformed service in compliance with all applicable federal and state laws. With exceptions and limitations, employees are entitled upon completion of military service to reemployment with the company and certain other benefits in accordance with applicable federal and state laws. Any employee who needs time off for uniformed service is to immediately notify his or her supervisor of the need for leave. The supervisor will provide details regarding the leave. If an employee is unable to provide notice prior to leaving for service, then a family member should notify the employee’s supervisor as soon as possible. You should be aware that our healthcare plan excludes medical care required as a result of military service, acts of war, insurrection, riots, or service in the armed forces of any government. Further, medical care and supplies furnished in the facility operated under the direction of or at the expense of the U.S. government or any of its agencies, or by a physician employed by such facility, is not covered; except, if you or your covered dependent are not entitled to such treatment without charge, eligible expenses will be considered by the plan.
