EXTERMINATOR SERVICES CONTRACT This agreement is entered into by and between       , (hereafter "Employer") and       , (hereafter "Exterminator"). NOW, THEREFORE, FOR AND IN CONSIDERATION of the mutual promises and agreements contained herein, Employer hires Exterminator, and Exterminator agrees to work for Employer under the terms and conditions hereby agreed upon by the parties: 1. Scope of Services. Employer agrees to hire Exterminator to perform the following service:       . 2. Term of Employment. Exterminator agrees to perform such services the performance of which shall begin from the date of signing this Agreement and shall be performed monthly every six months seasonally yearly as needed. 3. Exterminator is responsible for keeping the premises free from all bugs, pests, rodents etc. throughout the course of the year and shall be employed as an independent contractor to perform such duties. 4. Employer agrees to pay Exterminator, in consideration of the services contracted for, the sum of $       to be paid bi-weekly monthly as services are performed. 5. Exterminator shall be responsible for paying his/her own taxes, workers compensation, etc. 6. Contractor agrees to maintain at his/her own expense, comprehensive general liability insurance at a minimum amount o f $       , and to hold Employer free and harmless from any and all claims arising from any negligent act or omission under the operation of this Agreement. 7. This Agreement constitutes the entire agreement between the parties and shall not be supplemented, modified or amended unless executed in writing by both parties. 8. No waiver of any provision of this Agreement shall be deemed a waiver of any other provision. 9. This Agreement shall be construed according to the laws of the State of       . 10.
