For best results, please read and indicate that you understand and agree with the following information: 1. An airbrush tan, like any self-tanning product, will not protect your skin from burning in the natural sun or in a UV 1 tanning bed Please take the proper precautions when exposed to UV light. 2. For best results, we recommend Two (2) sessions within Seven (7) days to develop your desired color. Maintaining your color is possible with subsequent visits every Five (5) to Ten (10) days. Healthy hydrated skin will hold results longer and achieve color faster. Ask a tanning consultant for a recommendation of the best regimen for your skin type. 3. For best results, wait Six (6) to Eight (8) hours after being airbrushed before showering. This will allow your airbrush tan to fully develop and will ensure the optimal results. 4. The initial bronzers in the airbrush tanning solution may transfer to clothing or fabrics. The bronzers are water soluble; they will wash out of most fabrics. They may stain silk, lycra, spandex, nylon, wool or leather. 5. The airbrush technician is trained to provide a smooth even tan. However, due to normal skin variations, and session variables, it is possible you may occasionally find some areas of your tan slightly lighter than others. In most cases, these differences are minor and disappear after a few sessions. 6. Because of the moisturizing agents in the mist, your eyes may feel itchy or scratchy. The ingredients in the solution are normally harmless, and will not normally damage your eyes 7. All the components of the airbrush tanning solution have been safely used in cosmetics and nutritional products for decades and have been proven to be safe for the skin. If you have had adverse effects utilizing self-tanning products or moisturizers in the 1 Ultraviolet Light past, you may experience similar effects with the airbrush system. Please discuss this with your technician before proceeding. 8. We recommend that you hold your breath, during facial spraying. We make this recommendation, because most individuals would prefer not to breathe the mist. The solution is not normally considered harmful, in the minimal amounts that may be inhaled in a normal spray tanning session. 9. Many customers wear disposable shower caps to prevent the solution from settling into hair. Others tan without covering their hair. The solution normally won't penetrate the hair shaft and hair is generally unaffected. Though rare, some porous, course, or processed hair types, if uncovered, can be affected. 10. If you have any history of asthma or other respiratory or medical conditions that could be aggravated by the use of the Airbrush system, please consult a physician before proceeding. Please apprise your tanning technician of any past allergic reactions to DHA products, or other medical concerns. The undersigned understands and agrees to comply with all instructions set forth above. I have been properly instructed prior to my Airbrush Tanning Session. I am assuming any and all risks in using these services. I hereby authorize and direct employees or agents of _________________________________ (Name of Business) to perform such airbrush tanning procedures as may be deemed necessary or advisable, and have provided them with the above information as required. I acknowledge that the results of airbrush tanning do vary, and that no guarantees of specific results are offered or implied. For and in consideration of said being allowed to use the tanning facilities of _______________________________ (Name of Business), I release _____________ ____________________ (Name of Business), its directors, officers, employees and airbrush technicians (collectively “Releasees”) from all liability to me for any loss or damage to property or injury or death to me whether caused by Releasees or otherwise that result, directly or indirectly from the use by me of said airbrush tanning procedures and said tanning facilities operated by ________________________________ (Name of Business) . I further agree to indemnify, save and hold Releasees harmless from any loss, liability, attorneys’ fees, damage, or costs that they (or any of them) may incur arising out of or related to the use of said Airbrush and/or tanning facilities by me whether caused by the negligence of the Releasees or otherwise. I have inspected the tanning facilities and equipment and accept them as being safe and reasonably suited for the purposes intended; and D. I voluntarily signed this Waiver and Release.
