STATE OF VERMONT PROBATE COURT DISTRICT OF ____________ In Re the Estate of: ) DOCKET No. __________ ) __________________ ) , Deceased ) ) ) LATE OF ) ) __________________ . ) ) ) WHEREAS, you have been appointed trustee of the above under the Last Will and Testament of ____________ as allowed by this court; and WHEREAS, you have executed a bond to the Probate Court, to the satisfaction thereof, conditioned for the faithful discharge of the duties of a trustee, as required by the laws of this state: BY THE AUTHORITY OF THE STATE OF VERMONT, you are appointed trustee of the above-entitled estate and authorized and empowered to take charge of, manage and control all the trust estate, according to law and the will of the testator, a copy of which attached. You will make a true inventory of all the real estate, and of all the goods, chattels, rights or credits belonging to you as trustee, and which shall come to your possession or knowledge, and return the same to this court within 30 days from the date hereof. You will manage and dispose of all the estate and effects and faithfully discharge your trust in relation to the same, according to law and the will of the testator. You will render a full account of the trust property in your hands and the management and disposition of the same, and your receipts, disbursements and charge therein, as often as once each year during your appointment and other times when required by the court. At the expiration of your trust, you will settle your account with the court, and pay over and deliver all the estate and effects remaining in your hands or due from you on settlement, to the persons who shall be entitled to the same, according to law and the will of the testator.
