I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said Activity or Event. In consideration of my application and permitting me to participate in this Activity or Event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: A. Furthermore, I will, unless i am advised otherwise to do so, indemnify and hold harmless the following organizations: , including the event organizers, sponsors, and participants, from any and all claims, damages, losses or costs of any kind or nature (directly, causally, or otherwise) arising from my participation in the activity or event, including but not limited to, my (1) personal injury arising from my own physical injuries (including but not limited to, any mental injury, paralysis, and impaired mobility), or (2) property damage incurred in connection with my participation in this activity or event. If I am released from this liability under this waiver and release of liability form, it will not affect in any way my legal relationship, contractual status, or obligations to, or ownership or possession of, my property.
