The provision of this Waiver and Release will continue in full force and effect even after the termination of the services being provided to me, whether by agreement, by operation of law, or otherwise. I acknowledge, represent and warrant that I am not diabetic nor do I have poor blood circulation. I further acknowledge, represent and warrant that I do not have a abnormal skin condition, and I am not pregnant.
