I hereby give and agree to give you, my attorney or others such advice, counsel and direction as to the proper method, manner, place, time and means for, including with respect to my death, the avoidance of, or the provision of medical care and or treatment in the event of, my death and/or the establishment and conduct of my estate. 4.
