The use of the clubhouse is a courtesy to owners. The person(s) reserving the clubhouse
must take sole responsibility for meeting all deadlines and following guidelines. If for any
reason, the clubhouse becomes unusable after the date of this reservation, the Association
assumes no responsibility other than to notify you of the cancellation of your event.

use of the Clubhouse on the date, time and purpose stated my application.
________I am a Unit Owner in good standing and 100% current on my condo fees.

I agree that I will accept full responsibility for use of the Clubhouse during the time stated in the
application. I agree that the Clubhouse shall not be used for commercial purposes. Upon
termination of the event, I agree to return the premise and all property to the same condition as
when I received it. If it is determined that the premises need cleaning after my event, I agree to
pay a cleaning fee. If it is determined that repairs need to be made, I agree to pay for such
repairs. I agree that a decision of a Board representative that cleaning or repairs are needed will
be binding.
I agree that I will assume all risks of injury to myself and anyone else using the Clubhouse
with my permission during the time the Clubhouse is under my control. I agree that I shall
indemnify and hold harmless the _____________________ (Name) Condominium Association
and for any claim against them as a result of my use of the Clubhouse.
I agree there shall be no underage drinking of alcohol in the clubhouse and that any event at
which minors are present will be supervised by adults who will be on premises at all times

Regulations Agreement
Rules and Guidelines:
1. Anyone using the Clubhouse does so at their own risk. The Association is not
responsible for any accident, injury or loss incurred with in connection with the use of the
Clubhouse. Owner/Tenant making reservation agrees to indemnify and hold harmless the
Association, Property Management and all Unit Owners for any and all injury or damage to
guests or vendors, to person or property occurring in connection with this event.
2. The Clubhouse facility is under surveillance of camera. All Clubhouse activity is
taped in order to aid in the persecution of illegal activity or infractions of Association rules.
Violators will be prosecuted.
3. Reservations are nonexclusive. Other Fairways residents may use the clubhouse
during your event. You may not interfere with the rights, comforts, convenience, or peaceful
enjoyment of the Clubhouse by community residents during your event. If your special event
needs to be private , you should consider using another facility.
4. The reservation is restricted to the (e.g., upper level of the Clubhouse)

room, covered patio, kitchen and restrooms. The pool and pool area, fitness center and
tennis courts, TV room, library, WiFi area are not included in the reservation and may not
be used in conjunction with the reservation.
Reservations, Hours & Fees;
1. Reservation should be made 30 days in advance.
2. Clubhouse may be reserved no more that two times per week, and only one of the two
events shall be on the weekend (Friday, Saturday, or Sunday). Hours are from ___ a.m. to ___
p.m. No event shall extend past the closing of the Clubhouse at ____ p.m.
3. Owners limited to two events per year.
4. No smoking.
5. No pets.
6. No commercial usage of clubhouse.
7. Reservations are a first come, first served basis.
8. Until the Application and checks are received, your event is not officially reserved.
9. Guest parking is restricted to any of the open _____ parking spaces and circular drive
of the Clubhouse. No guest parking is allowed on any grass areas of the Complex Entrance or
on _________________ Blvd. Resulting damage to sod and the irrigation system will be
charged to owners.
10. No unsupervised minors shall be on the premises at any time.
11. Any music or noise shall be at a volume not to disturb the peaceful enjoyment of
residents.
12. Cooking out doors and BBQ's are not permitted.
13. No beer kegs.
14. No tape, tacks, staples or other materials, shall be used for hanging decorations.
Setting up for your event:
1. You may set up 1 hour prior to the beginning of your event and clean up should be done
within 1 hour after your event. No event including set up and clean up shall exceed
________ hours. Furniture in the great room can only be moved within the great room
and only during set up.
2. Please post notice of your event on the bulletin board three (3) days prior to your
event.
3. Anyone reserving the Clubhouse must supply their own cleaning supplies; trash
bags, paper products, etc.
4. Neither the Association nor its agents are responsible for any loss of personal
effects, dishes or food. Any items left at the clubhouse will be considered abandoned
and disposed of by the Association. A disposal fee may be charged.
5. The Association reserves the right to terminate any reservation which, in the sole
judgment of the Association, is inappropriate or results in unlawful conduct or activities
which violate the rules and regulations of the Association.
Security Deposit Refund Policy:
The deposit is refundable upon confirmation that the facility was left undamaged, clean, and in
good condition.
Post-Event Clean up and Inspection:
After your event, the premises will be inspected to verify the premises and all property and
equipment located there are in the same the condition that you received it.
