Letter from Known Imposter to Creditor Accepting Responsibility for Accounts, Charges or Debits- This form is used by a known imposter to accept responsibility for fraudulent accounts, charges or debits made in the identity theft victim’s name and to request that the debt be transferred from the victim’s name to the imposter’s name. This form is also used to request that the creditor confirm in writing to the identity theft victim that the creditor has removed the fraudulent activity from the victim’s records and has notified the credit bureaus to also remove such information from the victim’s credit reports. Letter Agreement Between Known Imposter and Victim to Work Out Repayment Plan- This form is an agreement between the known imposter and the identity theft victim in which the imposter agrees to accept financial responsibility for fraudulent activity, to work out a repayment plan, to sign a letter to the creditor(s) requesting that the creditor(s) transfer the debt from the victim’s name to the imposter’s name and reflect such change on the imposter’s, rather than the victim’s, credit report, and to commit to some form of counseling, such as mental health or financial counseling. Letter to Law Enforcement to Report Identity Theft by Known Imposter - This form is used by an identity theft victim who knows their imposter to notify their local, state or federal law enforcement agency of the known imposter identity theft and to request that the agency issue and forward to them a police report for identity theft. Letter to Credit Reporting Company or Bureau Regarding Known Imposter Identity Theft- This form helps identity theft victims who know their imposter correct fraudulent information in credit reports by notifying consumer or credit reporting companies of fraudulent information that is not related to any transaction that the victim made or authorized. Letter to Report Known Imposter Identity Theft to Postal Authorities- This form is used by an identity theft victim who knows their imposter to notify the local U.S. Postal Inspector of identity theft involving use of the U.S. mail and to request an identity theft report.
