Please issue to me at the address listed above a current copy of [enter name of deceased person] ’s credit report and place a “deceased” alert on [enter name of deceased person] ’s file. The alert should read “Deceased. Do not issue credit. If an application is made for credit, notify the following person(s) immediately: (List the names and contact information of the applicable next surviving relative, executor/trustee and/or local law enforcement agency representative and the relationship between the person(s) listed and the deceased person) .” Please also provide to me all contact information on the accounts currently opened in [enter name of deceased person] ’s name.
