1. Performer is hereby retained and shall perform the services set forth in Exhibit A ,
attached hereto and hereby made a part hereof (Services). The term Performer shall be
deemed to mean one or more Performers as requested by Client.
2. As full and complete compensation for Performer's Services, Client shall follow
compensation parameters listed below:
A. Client shall remit $_____________ deposit at least ______ (number) business
days prior to the agreed upon performance/event.
B. Performer shall pay balance of $_____________in full to Performer immediately
upon conclusion of the performance/event.

D. Often Performer will have more than one engagement per night, so it is important
that the show start at the agreed upon time. If the performer is required to start more

lateness may result in a shorter show or constitute a breach of contract on the part of the
Client.
E. Payment must be made in a timely manner once services have been rendered. If
Performer is delayed more than _______ minutes after the show for payment, an extra

3. Client shall provide:
A. A safe adequate space for Performer which includes a floor or stage free of
broken glass, spilled liquids, or other hazardous objects. The environment should be free
of harassment of a sexual nature or otherwise. If Performer deems the environment
unsafe or unacceptable for any of the above reasons, she may leave without rendering
services. In that case, Client will still be responsible for paying the full amount or the
charge.

C. Performer will need _________minutes prior to performance in a dressing room
to prepare. Client will insure that there will be a private, safe area with a mirror for the
Performer to change clothes and securely store her bags and belongings while
performing; plus restroom facilities in close proximity.
D. A sound system with CD playing capabilities, preferably located in the same
room designated for the performance and a qualified system operator shall be provided
by Client. Sound system must be tested prior to arrival of Performer .
E. Client shall communicate any special requests or suggestions at least ______
days prior to the date of the performance (including color/costume requests).

5. No Waiver
The failure of either party to this Agreement to insist upon the performance of any of the
terms and conditions of this Agreement, or the waiver of any breach of any of the terms and
conditions of this Agreement, shall not be construed as subsequently waiving any such terms
and conditions, but the same shall continue and remain in full force and effect as if no such
forbearance or waiver had occurred.
6. Governing Law
This Agreement shall be governed by, construed, and enforced in accordance with the
laws of the State of _______________.
7. Notices
Any notice provided for or concerning this Agreement shall be in writing and shall be
deemed sufficiently given when sent by certified or registered mail if sent to the respective
address of each party as set forth at the beginning of this Agreement.
8. Attorney’s Fees
In the event that any lawsuit is filed in relation to this Agreement, the unsuccessful party
in the action shall pay to the successful party, in addition to all the sums that either party may be
called on to pay, a reasonable sum for the successful party's attorney fees.
9. Mandatory Arbitration
Any dispute under this Agreement shall be required to be resolved by binding arbitration
of the parties hereto. If the parties cannot agree on an arbitrator, each party shall select one
arbitrator and both arbitrators shall then select a third. The third arbitrator so selected shall
arbitrate said dispute. The arbitration shall be governed by the rules of the American Arbitration
Association then in force and effect.
10. Entire Agreement
This Agreement shall constitute the entire agreement between the parties and any prior
understanding or representation of any kind preceding the date of this Agreement shall not be
binding upon either party except to the extent incorporated in this Agreement.
11. Modification of Agreement
Any modification of this Agreement or additional obligation assumed by either party in
connection with this Agreement shall be binding only if placed in writing and signed by each
party or an authorized representative of each party.
12. Assignment of Rights
The rights of each party under this Agreement are personal to that party and may not be
assigned or transferred to any other person, firm, corporation, or other entity without the prior,
express, and written consent of the other party.
12. Counterparts
This Agreement may be executed in any number of counterparts, each of which shall be
deemed to be an original, but all of which together shall constitute but one and the same
instrument.

Exhibit A
DESCRIPTION OF BELLY DANCE SERVICES
1. Services shall consist of providing a Belly Dance performance and/or Belly Dance lesson
or workshop. Changes in Belly Dance performance, lesson, or workshop schedule and/or
cancellation of Belly Dance performance, lesson, or workshop may be made by Client upon
prior notice of _______ business days to Performer . In the event Client fails to provide such
notice, Performer shall be entitled to invoice Client for the scheduled performance, and Client
agrees to remit payment in full. In the event that the performance has been paid for in advance,
Client will not be entitled to a refund.
2. Performer will make every effort to provide Client with a Belly Dance performance with
regard to date, time, duration, music, and props as requested by Client .
3. Please provide the following information:

events, including but not limited to: illness; injury; or the death of a loved one, which may
prohibit the fulfillment of the scheduled performance.
8. If appropriate, Performer will encourage audience participation through dancing and
tipping. Guests of honor may be invited to dance, but Performer will not force those who are
uninterested in participating
