That attached hereto and made a part hereof is a true and correct Account for t he period of ________________, to ________________. Said Account is a detailed itemized statement showing all receipts and disbursements made during said accounting period and showing assets on hand as of ________________, with a market value of $________________. 2. Said Conservator would further show that it has vouchers covering the disbursements shown in said Account attached hereto and made a part hereof, and that said vouchers are not being filed with said Account pursuant to Section 93-13-67, Mississippi Code of 1972, as amended, but are on file and shall remain on file with ________________ of ___________, Mississippi, and shall be available for inspection by any interested party. 3. That ________________ is the brother of the deceased mother of ________________ and is the closest relative of ________________ and is the only adult kin of said ________________ 2 within the third degree computed according to civil law.4. That ________________, was appointed as Co-Conservator of the person of ________________by Order of this Court on ________________, and Letters of Conservatorship were issued to ________________, on ________________. 4. That the law firm of ________________, has assisted said Conservator in carrying out its duties during the year of ________________, and to date, and has prepared and will present this Petition, and did prepare and present the petition and other pleadings necessary to have ________________ appointed as such Co-Conservator, and that said firm should be paid for said services in an amount to be determined by this Honorable Court. 5. That this Conservatorship Estate has received and will continue to receive contributions from the Trust under the Will of ________________, pursuant to the terms of said Trust, as necessary for the support and maintenance of ________________. Said Trust is being administered in Cause No. ________________ on the docket of this Honorable Court. WHEREFORE, Petitioner prays: 1. That said Account attached hereto and made a part hereof be received as the Twenty- Third Annual Account of said Conservator and that said Twenty-Third Annual Account be approved and allowed, and that all actions taken by said Conservator during the accounting period, a s reflected in said Twenty-Third Annual Account, be approved and confirmed. 2. That ________________ be allowed a commission of $________________ for its services as Conservator during the accounting period (________________, to ________________). 3. That the Conservator be authorized to pay the law firm of ________________, the 3 sum of $________________ for its services to the Conservators ,plus any reasonable out-of-pocket expenses which said firm has incurred in connection therewith.4.
