Medical Release Form – This Medical Release authorizes the physicians, hospital and all medical attendants to furnish full and complete med ical reports and information requested by the person signing to whomever such person designates in the agreement. This authorization also includes examination of all hospital records, x -ray film and furnishing of any information including opinions. Contra ct for Legal Services – This document states that the client employs the named attorney to represent the client and to institute proceedings with any appropriate court or institution for damages or other appropriate relief against such persons or entities. In considering the law office for legal services, the client agrees to pay the law firm a percentage of all amounts recovered on behalf of the client. General Release and Settlement Agreement - This form is a release and settlement agreement where the c laimant accepts a specified amount of money and forever releases and discharges the releasee from any and all claims. The form also provides that each party will pay their own respective legal fees. Petition for Pro -Ami Settlement - Minor – This form is u sed when the petitioners have agreed on the terms of a settlement which will settle all claims and discharge all parties from further liability arising out of this incident, and the petitioners ask the Court to conduct a Pro -Ami Settlement hearing to deter mine whether the proposed settlement is in the best interest of the minor child. Order for Pro -Ami Settlement – This form is used when the court agrees with the terms of the Pro -Ami Settlement, which will settle all claims and discharge all parties from f urther liability arising out of the incident, enter judgment and directs payment to be made. Pro Tanto Settlement and Release Agreement – This form is used when the parties desire to settle any and all claims and causes of action of any kind which have be en or could have been asserted in this action, arising out of the incident and dealings set forth in the complaint. It further states that no other claim of the plaintiff shall be affected by this Pro Tanto Settlement and dismissal. This form is availabl e in Word and Wordperfect formats. Pro Tanto Stipulation for Dismissal - This form is used when the claims have been resolved and the case has been dismissed with prejudice. The plaintiff may not bring forth any other charges on this same claim against t he defendant, and each party is responsible for their own attorney's costs, court expenses and fees. Motion to Appoint Process Server – This form is a request for the court to appoint a person authorized by law or by a court to formally deliver process to a defendant or respondent. Order Appointing Process Server – This form is a court order appointing a person authorized by law or by a court to formally deliver process to a defendant or respondent. This form is available in Word and Wordperfect formats. Notice of Deposition – This form gives notice of an upcoming deposition and the date, time, and location where the deposition will take place. This form is available in Word and Wordperfect format s. Pretrial Order – This form is filed prior to the beginning of a trial. It sets out the claims and defenses to be tried, the stipulations of the parties, and the case's procedural rules. It also states case information such as the attorneys for each p arty, reasoning for the suit, witnesses to be called at trial, etc. This form must be agreed upon and signed by both parties. Notice of Intent to Serve Non -Party with Subpoena – This notice of intent to serve a subpoena on nonparty is filed with the Circ uit Court and sent to the opposing party. The party serving this notice will then petition the court for an order allowing the service of the subpoena. Subpoena for Production of Documents – This form is an order to produce documents and materials for re view by the opposing party. Stipulation for Dismissal with Prejudice – This form is a used when both parties wish to dismiss the lawsuit with prejudice.
