1 Copyright 2004 USLegalforms.com PET ADDENDUM FOR _______________ This pet addendum to the lease between _______________ (Tenant) and _______________ (Management) is made part of the lease entered into between the parties on _______________. 1. Both parties have read and signed the pet policy in effect for the premises which is attached to said addendum. 2. The Tenant agrees to keep all pets in a responsible manner and provide proper care for them. 3. The Tenant shall be liable for any and all damages or injuries caused by his/her/their pet(s). If the tenant’s security deposit does not cover the damages, management and the tenant will agree on a payment plan to pay for the damage as well as replace the security deposit. 4. The Tenant shall show proof of spaying or neutering, licensing, and inoculations if such procedures are required for the animal(s). 5. Tenant shall provide the following information (to be updated annually): Pet Caretaker Name: Address: Telephone: Veterinarian Name: Address: Telephone: If the Tenant is unable to provide the name of a pet caretaker, he/she/they has made the following arrangements for the care of the pet(s): 6. Tenant shall keep the pet(s) in a manner that is in keeping with the cleanliness standard of the residence. Tenant shall properly clean up after a dog and properly dispose of cat box filler and litter from birds and rodents. 7. Tenant shall not leave the pet(s) unattended outside his/her apartment. 8.
9. The management has authorized us to enforce the provisions of item #6 above. 10. Management is responsible for any and all damages or injuries caused to the pet(s). If the pet damages the premises or any of your pets, please notify our office as soon as possible. If the pet is a pet you own and the pet caused damage, you must notify our office. 11. The management will provide a written receipt. 12. Management shall not disclose the names or addresses of pet caretakers as a condition for housing in our building. 13. Pets are not allowed in any of the following areas: 1. Bedrooms and bathrooms 2. Play areas 3. Laundry room 4. Laundry floor 5. Dining area 6. Laundry tables and chairs 7. Outdoor play area 8. Play area and parking lot 9. Outdoor fountains or pools 10. Outdoor kitchen 11. Outdoor lounge room 12. Outdoor patio and patio 13. Garden 14. Grass areas 15. Yard 16. Laundry 17. Maintenance building 18. Playground 19. Parking lot at the foot of the building. 21. Any place which is prohibited by local ordinance, lease, or other agreement. 22. Maintenance building 23.
