The form(s) in this packet may contain “form fields” created using Microsoft Word or Adobe Acrobat (“.pdf” format). “Form fields” facilitate completion of the forms using your computer. They do not limit your ability to print the form “in blank” and complete with a typewriter or by hand. It is also helpful to be able to see the location of the form fields. Go to the View menu, click on Toolbars, and then select Forms. This will open the Forms toolbar. Look for the button on the Forms toolbar that resembles a shaded letter “a”. Click this button and the form fields will be visible. By clicking on the appropriate form field, you will be able to enter the needed information. When completing the form please take note of the following requirements: 1. The LAST NAME of the grantor and grantee must be capitalized or underlined wherever first mentioned in the deed. 2. The complete business address of the grantee must appear in the form fields provided in the left margin. 3. The tax map reference in the left margin must be completed. 4. The consideration and the assumption balance, if applicable, should be inserted in the appropriate form fields in the left margin of the first page. II. MANDATORY COVER SHEET REQUIREMENTS FOR RECORDING LAND RECORDS Pursuant to Code of Virginia § 17.1-227.1 ,, any County Circuit Clerk’s Office may exercise the option to require a Land Records Cover Sheet for all documents recorded in the Deed Book. In practice, data contained on Cover Sheets is converted by a software program into bar codes which are scanned via bar code readers, thereby allowing instruments to be recorded more timely and accurately. To create the required Cover Sheet, users must access Cover Sheet software made available at the following web site: CSA Online website . Documents that do not include a Cover Sheet with all information accurately listed will be returned to the presenting party so that a corrected Cover Sheet may be prepared. Questions related to procedures should be directed to the local Clerk’s Office. To determine if the Clerk in the County where the property is located requires a cover sheet, you will need to contact the Clerk's office. III. SELF-ADDRESSED STAMPED ENVELOPE TO ACCOMPANY THE DEED WHEN RECORDING: The Clerk's Office will require a SASE if you want the deed returned by mail after recording is done. If a SASE is not provided, the recorded instrument will have to be picked up at the Clerk's office. IV. IF A DESIGNATED BENEFICIARY FAILS TO SURVIVE THE TRANSFEROR / OWNERS: If the transferor / owner has identified two or more designated beneficiaries to receive concurrent interests in the property, the share of one that lapses or fails for any reason is transferred to the other, or to the others in proportion to the interest of each in the remaining part of the property held concurrently. For example: Deed providing - A and B to C, D, and E upon their deaths. If C fails to survive either or both A and B, upon the death of the survivor of A and B, the property would go to D and E in equals shares. IV.
