This form asks the cour t fo r a new hearing date and makes your orde rs l ast until then. Ask the clerk for the f orm. After the court has reissued the orders, yo u mus t a t tac h a c o p y o f CH- 1 2 5 t o a co p y of y o ur ori g i n al o rders.
