Agreement for Conditional Gifts Conditional Gift Agreement entered into this __________________ (date) between _____________________________ (Name of Donee) of _____________________________________________ (street address, city, county, state, zip code) , referred to herein as Donee, and _____________________________ (Name of Donor) , a limited liability company organized and existing under the laws of the state of _____________________________ (name of state) , with its principal office located at _____________________________________________ (street address, city, county, state, zip code) , referred to herein as Donor. Whereas, Donor desires to make cash gifts to Donor until __________________ (date) for apartment or house rent, a certain sum for living expenses, and for part of the college tuition expenses of _____________________________ (Name), the __________________ (son/daughter) of Donee; and Whereas, these gifts will be made for one year provided certain requirements are met; Now, therefore, Donor and Donee agree to the following terms and conditions: 1. Beginning __________________ (date), Donor will pay the rent for Donee’s residence in an amount not to exceed $ __________________ , provided only Donee, __________________ (Name) and __________________ (Name) reside in said residence. 2. Beginning __________________ (date), Donor shall pay to Donee, on a bi- weekly (once every two weeks) basis, the amount of $ __________________ for other living expenses. 3. Beginning __________________ (date), Donor shall pay to Donee, a sum not to exceed $ __________________ for the college tuition of _________________________ (Name). 4. Payment of the aforesaid sums are conditioned upon the following: A. Donee shall provided to Donor a monthly report (in a form satisfactory to Donor) on __________________ (Name) ’s medical condition commencing on __________________ (date). B. __________________ (Name) must provide a monthly report (in a form satisfactory to Donor) of his/her progress in college and a final transcript each semester. C. __________________ (Name) will meet with Donee once per month to ensure that Donee’s residence is adequately taken care of and that no other services will be needed. D. Upon the execution of this Agreement, Donee will provide Donor with a copy of his/her lease agreement to his/her residence. 5. Funds of Donor to fulfill this Agreement shall be deposited in account number __________________ of __________________ (Name of Bank) located at _____________________________________________ (street address, city, county, state, zip code) .
