IN THE CHANCERY COURT OF COUNTY, MISSISSIPPI IN THE MATTER OF THE CONSERVATORSHIPOF CAUSE NO. PETITION TO APPOINT NEW CONSERVATOR COMES NOW the Petitioner , and files this her Petition to Appoint New Conservator of the person and estate of and in support thereof would respectfully show unto the Court the following: 1. is an adult resident citizen of County, Mississippi and is the sole surviving child of the Ward. 2. is an adult resident citizen of County, Mississippi, presently residing in the whose address is , Mississippi. The health of the Ward has deteriorated since the Conservatorship was opened and the Ward is still in need of constant care and supervision being provided by the and the need for this Conservatorship still exists. 3. On or about , 20 , , , Mississippi was appointed Conservator of the estate and person of . is the daughter-in-law of the Ward and was substituted and appointed as Conservator after the death of her husband and original Conservator, 4. The present Conservator resides in County, Mississippi and the Ward is presently residing in County, Mississippi. Petitioner would show that the present Conservator is not in a position to adequately attend to the daily needs of the Ward and to make regular visits to the where the Ward resides. 5. Petitioner would show that it would be in the best interest of the Ward if she were appointed as Conservator due to the fact that she is the sole surviving child of the Ward and resides in the same county as the Ward. Petitioner is capable and qualified to assume the duties as Conservator of the estate and person of the Ward and is willing to serve as same. 6. Petitioner would show that an Annual Accounting was filed by the present Conservator on , 20 which gave an accounting from , 20 to , 20 . Petitioner requests that the Court order a supplemental accounting covering the time period from , 20 through the date of the appointment of the new Conservator. Petitioner requests that the accounting be filed with the Court within thirty (30) days of appointment of the new Conservator. 7.
