Plaintiffs John Doe and Jane Doe are husband and wife and they are both citizens of the United States of America and both reside and are domiciled at the above cited address. 2. Plaintiffs desire to adopt Mary Doe, who was born on [insert] in [insert hospital] in [insert city and state]. Mary is currently a little over five months old. 3. Mary is the grandchild of the plaintiffs. 4. The plaintiffs have the following children: [insert each child = s name and date of birth]. 5. The plaintiffs received the child, Mary, to be adopted from their daughter, Michele Doe, who is the birth mother of the child. 6. The child came under the continuous care of the plaintiffs on [insert date], and Mary has continued to be under the care of the plaintiffs until the present time. 7. The Complaint was not filed until now because the plaintiffs wanted to insure that Michele Doe would consent to this adoption and that the identity of the birth father was confirmed. 8. The name of the birth mother of the child is as stated above. 9. The birth father is unknown and in fact Michele Doe had previously testified under oath that the identity of the birth father is unknown. See Exhibit 1 (insert transcript information). 10. The plaintiffs request that the Court waive services of Complaint on the birth father pursuant to [insert statute]. The statements under oath by the birth mother as to the birth father support this request. 11. The child = s birth mother is presently unmarried. 12. The consent of the birth mother to this proceeding has been given and [insert consent order information] exhibit 2. 13. The child to be adopted has no property except for clothing, toys and other necessities. 14. The name by which the child to be adopted shall be known is Mary Doe. 15. The plaintiff Jane Doe is employed as a Municipal Clerk for the [insert place] and is presently a crossing guard in [insert place and state] The plaintiff John Doe is retired. The plaintiffs represent that they have the wherewithal and ability to support the child to be adopted.
