VOTING REGISTRATION REQUIREMENTS - In order to register to vote in the state of Louisiana, you must meet the following requirements:  You must be a citizen of the U.S.  You must be a resident of the state and parish in which you seek to register and vote.  You must be at least 17 years old to register but must be 18 years old to vote.  You must not be under an order of imprisonment for conviction of a felony.  You must not be under a judgment of full interdiction or limited interdiction where your right to vote has been suspended.  You must register to vote by completing a Louisiana Voter Registration Application form 30 days prior to an election to be eligible to vote in that particular election. You may register in person at your local Registrar of Voters offices or you may obtain a mail-in application online at http://www.sos.louisiana.gov/elections/elect-appli.htm . You may also register to vote while seeking services at the Louisiana Department of Motor Vehicle offices; Louisiana Department of Social Services offices and WIC offices, Food Stamp offices, and Medicaid offices; Offices serving persons with disabilities, such as the Deaf Action Centers and Independent Living offices; and Armed Forces Recruitment offices.  If you register in person, you are required to prove your age, residency, and identity. If you register by mail, you will receive a verification mailing card to verify the address you provide. IDENTIFICATION CARD REQUIREMENTS - In order to obtain an identification card in Louisiana, you must meet the following requirements:  You must provide one primary and two secondary identification documents to your local Office of Motor Vehicles. o Primary documents include the following:  A certified copy of birth certificate (long form), original certificate of birth, birth registration card or certificate issued by a state or county bureau of vital statistics with raised agency seal, birth card (short form).  Passport- Foreign passports must be appropriately stamped and accompanied by proper immigration documents (I-94).  Permanent Resident Card (I-551) - If the photograph on the I-551 was taken prior to your 14th birthday and you are now over age 14, you must be referred to the Department of Homeland Security for issuance of a new I- 551 prior to processing the application.  Certificate of naturalization.  Valid U.S. military identification card or draft record or military dependent identification card.  Current U.S. Coast Guard Merchant Mariner card.  Certification of birth abroad issued by the U.S. Department of State.  Native American tribal document.  Louisiana Department of Public Safety & Corrections prison or parish I.D. card containing photograph, name, race, sex, and date of birth.  Department of Homeland Security Refugee travel document (I-571).  Department of Homeland Security Permit to reenter the U.S. (I327).  Employment/work authorization document issued by the Department of Homeland Security (I-688B or I-766). o Secondary documents include the following:  Out-of-state driver's license with a photo which clearly identifies you (or an official copy of driving record from last state of issuance if you have lost your out-of-state license.)  Social Security card or official written documentation verifying your Social Security Number from the Social Security Administration or specifying that no number has been assigned or that you are ineligible for a Social Security Number.  Out-of-state photo identification card issued by a state motor vehicle department.  Student I.D. card for a Louisiana college or university when accompanied by a 100% fee paid receipt for the current semester.  Original documents of adoption papers.  Original of a high school, college or university diploma.  A certified copy of a marriage license or certificate of marriage or an original or certified copy of a divorce issued by a county, parish or city in the U.S.  Church Baptismal Certificate.  Louisiana voter registration card.  Official certified deeds or title to property in Louisiana, including burial plot deeds.  Vehicle registration or certificate of title of vehicle in your name.  Local utility statements showing your name and address or receipt showing that public utilities have been turned on.  Insurance policy (health, home, life, auto).  One payroll stub (Cannot be handwritten).  Temporary resident card (Form I-688).  High school year book – if picture clearly identifies you.  Medicare/Medicaid card or medical eligibility card.  Prison release documents or letter from probation officer.  DD214 (military discharge papers).  Selective Service notification to you at your address.  Original or certified copy of any professional degree, certificate or license.  School records or two report cards from separate years.  Motor vehicle lien instrument.  Any government law enforcement officer's identification or badge, if in uniform, a police unit, etc. (does not include private security company guards).  Letter of verification/introduction from another state agency responsible for placement of deprived or handicapped persons, i.e. Blind Services.  Employment ID with photo if photo clearly identifies you.  Identification card issued by federal, state, or local government agency or entity if it contains a photo which clearly identifies you and information such as your name, date of birth, sex, height, and address.  W-2 forms for two years.  Certificate from the Department of Education for driver's education course completion.  Six-Hour Pre-licensing Completion Certification.  Completion Certificate from the 36 hour Pre-licensing course.  Original "Refugee Resettlement Program" letter with color picture of you.  If you are a minor 17 years of age or younger, you must be accompanied by a parent or legal guardian with a picture I.D. In case of joint custody, only your domiciliary parent may sign.  You must pay a fee of $21.00, unless you are age 60 or older. DRIVER’S LICENSE REQUIREMENTS - As a new Louisiana resident, you are required to transfer your out-of-state driver’s license to Louisiana within 30 days. If you are a full time student living in Louisiana but maintaining out-of-state or out-of-country permanent residency, you do not have to obtain a Louisiana license, as long as your full-time student status is in effect and your driver’s license is valid. If you are military personnel or a dependent assigned to Louisiana, you are not required to obtain a Louisiana license. In order to obtain a driver’s license in Louisiana, you must meet the following requirements:  You must appear in person at a local Office of Motor Vehicles to apply for a Louisiana driver’s license and present the following: o One primary identification document from the above list. o Current, valid out-of-state driver’s license, official copy of your driving record, clearance letter, or teletype from law enforcement faxed directly to your local office. o Social security card. o Proof of insurance if you own an automobile.  If the name on your driver’s license does not match the name on your birth certificate, you must furnish your state certified marriage license.  If you are a minor, 17 years of age or younger, you must be accompanied by a custodial parent or legal guardian with picture I.D. In case of joint custody, only your domiciliary parent may sign.  You must meet visual and medical requirements. If you transfer a valid, non-commercial, out-of-state license, you will only be required to take the vision test. You will have to take the written test if your out-of-state license is expired one year or more. You will have to take the written and driving skills tests if your out-of-state license is expired two years or more.  You must surrender your out of state license.  You must pay a fee in the amount of $24.50. V EHICLE TITLE AND REGISTRATION REQUIREMENTS - As a new Louisiana resident, you must register your vehicle within 30 days of establishing residency. In order to register your motor vehicle in Louisiana, you must submit the following items to the Office of Motor Vehicles:  Application for title and registration (DPSMV 1799).  Payment for fees and/or taxes.  Proof of military service if you are military personnel.  Original out-of state certificate title in your name or a photostatic copy if the original is being held by a lien holder.  Current registration certificate.  Proof of payment of sales tax on the vehicle.  Proof of current liability insurance in the form of one of the following documents: o Copy of your insurance policy o Binder or a copy of your I.D. card issued by the insurance company o Copy of the declaration page, or written statement from the insurance company or agent on letterhead stationery, signed by a representative of the company/agent that includes a description of your vehicle and your vehicle identification number.  If there is a lien on your out of state vehicle, you must present one of the following documents: o A non-negotiable copy of the out-of-state title showing the lien o A valid registration certificate indicating the owner and lien information o A letter from the Motor Vehicle authority of the state which issued the title confirming the name of the owner, lien information and description of the vehicle o A copy of the "title application" filed in another state when that copy indicates that there is a lien and when that copy is validated by a Motor Vehicle or Revenue employee, except if your title application is from Mississippi o A Vehicle Application (DPSMV1799) form completed and signed by the owner, or a computer print out signed by the owner.
