VI. By written notice dated, and served on Defendant on ________ (date), in compliance with ________________________ (cite applicable state statute) , Plaintiff terminated Defendant's tenancy and demanded that Defendant quit the Premises and surrender possession to Plaintiff within _____days after service of the notice.
