The parent to deliver the form and the form's contents are a condition precedent to the consent to the adoption and constitute an instrument, a document which, while it shall have force and effect, has no legal force and effect. The form is a copy of the adoption order, which is available from the court or probate court that granted the adoption. The form is not admissible into evidence.
