Waiver and Release from Liability for Injuries Sustained in Art School The undersigned _________________________ of _____________________________ _________________________ (street address, city, county, state, zip code) , does hereby waive and release, and will indemnify, hold harmless and forever discharge _________________________ , d/b/a/ ________ Art School located at __________________________________________________ (street address, city, county, state, zip code) , including his agents, employees, officers, directors, affiliates, successors and assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to my participation in any of the activities in which I participate while taking art lessons at _______ Art School, including any related activities conducted by or on the premises of ______ Art School. This waiver of liability does not apply to any acts of gross negligence, or intentional, willful or wanton misconduct. _________________________ , d/b/a/ _________ Art School, his agents, employees, affiliates, successors and assigns are herein referred to jointly and severally as ____________________ (school). I understand that the activities that I will participate in will involve the use of sharp objects for cutting and soldering. Soldering is a method of joining metal parts using an alloy of low melting point below 450 °C (800 °F). Heat is applied to the metal parts, and the alloy metal is pressed against the joint, melts, and is drawn into the joint by capillary action and around the materials to be joined. The use of sharp objects and soldering are only two examples of activities in which I will participate that can be dangerous and may cause me to suffer serious bodily injury including death. On behalf of myself, my heirs, assigns and next of kin, I waive all claims for damages, injuries and death sustained by me that I may have against _________________________ regarding any such activity. By this Waiver and Release, I assume any risk, and take full responsibility and waive any claims of personal injury or death associated with receiving art instruction from _________________________ or other related activities conducted by _________________________ . This Waiver and Release contains the entire agreement between the parties, and supersedes any prior written or oral agreements between them concerning the subject matter of this Waiver and Release. The provisions of this may be waived, altered, amended or repealed, in whole or in part, only upon the prior written consent of all parties. The provision of this Waiver and Release will continue in full force and effect even after the termination of the activities conducted by, on the premises of, or for the benefit of _________________________ , whether by agreement, by operation of law, or otherwise. I have read, understand and fully agree to the terms of this Waiver and Release. I understand and confirm that by signing this Waiver and Release, I have given up considerable future legal rights.
