Timesheet Instructions Every employee and contractor of _____________________ (Name of Employer) is required to complete and submit a timesheet weekly. Each line of the timesheet must contain all required information. Timesheet entries should be completed on a daily basis. For eligible employees Paid Time Off must be recorded as it is actually used. Ditto marks are not acceptable. All timesheets must be typed or completed in blue or black ink. All timesheets must be signed. Accurately recording time worked is the responsibility of every employee. Examples might to specify type of time off, such as Holiday, PTO (paid time off), or Unpaid Time. Hours Worked: Enter the correct number of hours worked for each day. For eligible employees enter all Paid Time Off as it is actually used.
