[Music] hello and welcome to our educational video medicare home health eligibility criteria the face-to-face encounter the five criteria that render a patient eligible to utilize their medicare home health benefit are listed on your screen the patient must be confined to the home otherwise known as homebound have a need for skilled services remain under the care of a physician or non-physician practitioner receive services under a plan of care established and reviewed by a physician or allowed non-physician practitioner and the patient must have had a face-to-face encounter for their current diagnosis with a physician or a loud non-physician practitioner the intent of this short educational session is to ensure that our providers understand the eligibility criteria regarding the face-to-face encounter so what is a face-to-face encounter exactly it is the actual medical record documentation created with a physician's office an acute or post-acute care facility created by the physician
