Individual Caregiver’s Service Agreement 1. Client further acknowledges he/she understands that he/she may revoke this agreement by written agreement upon written notice to caregiver at any time, except for purposes of terminating this agreement, on the basis of no longer being able to provide the services specified herein, or on the basis of any other cause the parties may agree upon, and that any such revocation must not be effective for more than one (1) year from the date of initial implementation. 9. Client and caregiver also agree that: (i) client will pay caregiver directly for all services provided under this agreement, (ii) each party agrees to reimburse caregiver for all services provided under this agreement, and (iii) the parties hereby waive any right, claim, or cause of action to any other person or persons except client, caregiver, or their attorneys, and shall not enter into any settlement, arbitration, agreement, or litigation in any relation to the subject of this agreement or cause this agreement to be construed as a waiver thereof. 10. Client acknowledges that all statements contained therein are true and accurate to the best of either party's knowledge and belief, and if any statement contained in this agreement cannot be verified by the other parties, this agreement is void. 11.
