(c) disclosure by lessee (check (i) or (ii) below): (i) lessee has received all applicable disclosures required of renters on behalf of a lessor, but has not received the disclosure of a lead-based paint hazard for the premises or in the property (explain). (ii) lessee is in compliance with the disclosure requirements of this section (check (i) or (ii) below). (d) lessee has not violated any section of the lead-based paint disclosure act. (e) lessee has not failed to report compliance with the disclosure requirements of this section. (f) lessee has not failed to maintain the required records or reports. (g) lessee has not failed to notify the United States of the existence of any hazardous substances contained in the premises (check (i) or (ii) below). Leases. A lessor must inform a lessee of the lease agreement under which the lessor is obligated to accept a lease, and provide notice of the provisions of, and responsibilities for, the lead-based paint disclosure requirement (see section (h) below). In addition, a lessor must either (a) include on a leasing statement the disclosure required by this section and/or (b) provide a separate disclosure statement to each lessee of a portion of its leasing portfolio. An additional lease disclosure statement for units subject to a rental agreement and/or rental inspection would be permissible, provided there is adequate disclosure in a separate lease disclosure statement. § minimum requirements. (a) the disclosure requirement for a lease or rental agreement is no more than one of the following statements: (1) the premises have lead-based paint or lead-based paint hazards identified (check (ii)). (i) if, during the term of the lessor's lease or rental agreement, the lessee fails to use a qualified hazardous waste disposal facility to dispose of, or remove from the premises, a material that contains significant amounts of lead, any lead-based paint or lead-based paint hazards identified, then the lessee would be subject to civil penalties equal to one-half of the amount allowed as a civil penalty for violation of §
