residence is in Utah and you intend to continue to reside in Utah
indefinitely.
You must be at least 18 years old on or before the next election.
You must register to vote at least 30 days before an election by one of the
following methods:
o Download and print a Utah Registration Form at
http://elections.utah.gov/VoterRegistrationForm.pdf and mail the
completed form to your County Clerk’s Office. If possible, you
should include a photocopy of your driver's license or another form
of picture identification to avoid having to present this information at
the polls.
o Photocopy a voter registration form from the White Pages of your
phone book and mail the completed form to your County Clerk’s
Office.
o Register in person at your local satellite voter registration location.
o Pick up a registration form at a State Agency Office, including the
Drivers License Division, Division of Workforce Services, Utah
State Department of Health, Women, Infant and Children (WIC)
Offices, the Division of Services for People with Disabilities or the
Department of Rehabilitation, or at a public college or university.
IDENTIFICATION CARD REQUIREMENTS -
In order to obtain a Utah identification card, you must meet the following
requirements:
You must submit proper identification (as discussed below for driver’s
licenses), proof of residence address (as discussed below for driver’s
licenses) and proof of your Social Security number. If your name has
changed, you must also present a marriage certificate or court document.
You must pay a fee of $8.00.
If you are under the age of 16, your parent or legal guardian must
accompany you to obtain an identification card.
DRIVER’S LICENSE REQUIREMENTS -
In order to obtain a driver’s license in Utah, you must meet the following
requirements:
If your driving status in another state is not valid, you must clear your
driver’s license action(s) before you can apply in Utah.
You must surrender your out-of-state driver’s license. If you do not have
your out-of-state license in your possession, you must provide proper
identification and proof of resident address. The Utah Driver License
Division must be able to verify your current driver’s license by contacting
your prior state to verify a "valid" driving status. You must pass an eye test
and written test. If your out-of-state license has lapsed, you may also be
required to pass the driving test.
o You must show proof of residence address in the form of at least
two different types of original documents from the following list
identifying your name and residence address:
Property tax notice, statement or receipt, within 1 year
Utility bill, billing date within 60 days (no cell phone bill)
Utah Vehicle registration or title, current not expired
Bank statement, within 60 days
Recent mortgage papers
Current residential rental contract
Major credit card bill, within 60 days
Court order of probation, order of parole or order of
mandatory release, must display residential address on the
order
Transcript(s) from an accredited college, university, or high
school.
Other documentation that unequivocally demonstrates proof
of residency or domicile.
You must be 16 years old to apply for a first time Utah driver’s license and
you must furnish proof of the following:
o A prior driver’s license or proof of Driver Training (Driver training
certificate).
o Proof of resident address (as discussed above).
o Proof of legal presence (if applicable) in the form of one of the
following:
U.S. State, Local Government-issued Birth Certificate.
Valid U.S. Passport.
Valid Foreign Passport with Appropriate Immigration
Documents (Must include or be accompanied by an I-94).
Certificate of Citizenship (N560).
Certificate of Naturalization (N550).
INS I-551 Resident Alien Card issued since 1997.
INS I-688 Temporary Resident Identification Card.
INS I-688B, I-766 Employment Authorization Card.
U.S. Department of Receptions and Placement Program
Assurance Form (Refugee).
o Proof of your name, date and place of birth from the following list of
documents, at least one form must be a Primary Form of
Identification:

VEHICLE TITLE AND REGISTRATION REQUIREMENTS -
For vehicle registration purposes, you will be considered a resident of Utah and
be required to register and title your vehicle, if you engage in a trade, profession,
occupation or gainful employment in Utah for more than 60 days.
In order to title and register your vehicle in Utah, you must meet the following
requirements:
You must bring to the Division of Motor Vehicles the following:
o A completed Form TC-656 Application for Utah Title, if your vehicle
is being titled and registered in Utah for the first time.
o Your vehicle’s most recent registration.
o Your vehicle’s title.
o Utah Safety Inspection Certificate and Utah Emission Certificate, if

o Vehicle Identification Number (VIN) inspection, if your vehicle is
