follow the link right now we'll see you at letta clearance for Florida is the one we're gonna want to select and we'll be prompted with the screen here at the beginning or in the login screen so if we do not have to count we can register with our email and a password if we do an account we'll just sign in our login screen and then we'll start our clearances here so now we're at the main clearances screen we'll select our year which gonna be the 2019 and 2020 year right now in the future the next school year will choose the 2020 and 21 year all right now let's stick with the 2019 after that we'll select our school and then we'll select our sport we'll be able to select multiple sports later and that will be brought to step number one which is our student information if we already have a student in the fridge in the system we can select this one for all of our students if not we'll create a new one so basically we're gonna be filling out anything that has a red asterisk for name birthday gray gender student ID the student ID is gonna be emailed to the all the athletes and their parents so be on the lookout for that one shortly right now we're just gonna put in a random number well input our student dress or our home phone number our city state and our zip code after we finish that we'll see our students family physician or doctor and their phone number and then also the education industry on the bottom the education history is for any students attended different high school has never attended different high school so anyone from 10th 11th or 12th grade a student who's entering a grade as a freshman or students transferring from another school select this option if you are transferring from another school download the affidavit of compliance sign it and upload it through the athletic clearance website as I'll show you a little bit later another detail select the school you're coming from so let's select that we're coming from 9th grade in step number one we have the fiscal forum so let's click this one and show you what the physical form looks like under this page the el2 looks like this you will bring this to the doctor and the doctor will sign you clearing you for participation they will see have a signature right here as well as the date and this will be uploaded or scanned to the athletic clearance website no paper copies should be submitted to the front office or any of the athletic directors it should only be filled out online I will also attach a video in form to show how to scan from an iPhone and Android app so you can email to yourself and upload it from there under additional forms we can choose that ga4 or the transfer form and upload it from here under step number two we have the medical history the medical history is essential to for our athletic trainer to see any allergies concussions or any surgeries that have been done so we can best help the student-athletes right now we'll check yes and we'll see that we can list all of our allergies after that we can click no for the rest and we'll follow the next step after this now that we have finished our medical history we will go to the parent guardian info under here we'll enter our first or last name for the parents as well as their business mobile and email there's another section for the second parent if they want to put that one in as well or a guardian right now we won't select that one and we'll see who's filling out this form either the athlete or parent or guardian under the college recruiting process the NCSA offers a free recruiting profile if you are interested this is not to be confused with the NCAA so we're gonna select a no thank you and we're brought to a signature screen this signature screen is the el3 or the consent form so that one does not have to be uploaded just digitally signed parents and athletes read through these and sign on the left after these are signed we'll move onto the next step as you can see we signed all of our information here and moving on to the next one under this one it's a donation from other athletics do not feel obligated to donate so now we'll skip to the next confirmation screen after you to this page you'll see that you have completed the registration for your student to play a specific sport you'll scroll down and see that this page you've asked for parents students signature this is not required as we do not want to waste paper you can scroll down to select the additional sports your son or daughter would like to play after selecting those and acknowledging with your signature
