Open Certificates MMC - right click on CA Certificate (or whatever). Select New Certificate Signing Request. Enter the hostname, the subject, the issuer, and the serial number on a separate line, then click Next. Enter the certificate store path, and click Next. Enter the name of the CA certificate in the Certificate store location of your choice, and click Next.
The certificate request window appears, and you may also configure the client computer to trust the signed request, by right clicking on a blank space in the request, and selecting Trust this certificate request from a list.
After the signing request is signed and sent to the certificate authority, you should see the signature on your server computer:
