of the subscribing witnesses, there appeared:

which declared that it is the holder of a certain promissory

date of [_Date_1_] , which said mortgage is recorded in
Mortgage [_Recordation_1_] of the public records of the Parish
of [_Recordation_Parish_] .
Said Lender further declared that the said [_Borrowers_]
have this day by act executed before [_Notary_2_] , Notary
Public, granted a mortgage in favor of [_Payee_2_] upon the
identical property which is subject to the mortgage referred
to in the last preceding paragraph (being
[_Property_Reference_] ), which said mortgage in favor of
[_Payee_2_] is to secure a note in the principal sum of
[_$Amount_2_] , payable at [_Place_Payable_2_] , due [_Terms_2_]
as is more fully described in that mortgage recorded in
Mortgage [_Recordation_2_] of the public records of the Parish
of [_Recordation_Parish_] .
And said Lender further declared that it desires to
subordinate in favor of the mortgage executed this date, the
mortgage dated [_Date_1_] and recorded at Mortgage
[_Recordation_1_] , and for that said purpose it does hereby
consent and agree that the mortgage executed [_Date_1_] and
recorded at Mortgage [_Recordation_1_] shall in all respects
and for all purposes be subordinate and inferior to the
mortgage dated this date and recorded at Mortgage
[_Recordation_2_] ; it being the intent and purpose of said
Lender that the said mortgage dated this date and recorded at
Mortgage [_Recordation_2_] shall be and at all times remain a
first mortgage upon the property therein described, and that
the mortgage dated [_Date_1_] and recorded at Mortgage
[_Recordation_1_] shall be and at all times remain a
subordinate mortgage upon the property therein described, and
in the event of a sale of the mortgaged property, by
foreclosure or otherwise, said note dated this date and
paraphed for identification with the act of mortgage recorded
at Mortgage [_Recordation_2_] shall be paid by preference and
priority.
And said Lender does now declare that it does authorize
and direct the recorder of mortgages for the Parish of

subordination upon the mortgage recorded at Mortgage

me, Notary, the mortgage note dated [_Date_1_] held by Lender

this act, was delivered by me, Notary, to said Lender who
hereby acknowledges the receipt thereof.
