welcome to the patient portal training series in this video we will cover updating your current information patient history and viewing your medical history click into the profile section located at the top of your portal screen to access your patient information here you will have the opportunity to update your current information as well as your patient history all information input here will be sent to your provider to update your chart and provide necessary information for treatment starting in current information any data your provider currently has for you in your electronic chart will display here you can update any field such as your address and phone numbers specify whether or not you'd like to receive appointment reminders via text or email and set your marital status and emergency contact information scroll down the form to fill in additional demographic details such as your school or employer race and ethnicity and more if applicable you can also include information about your spouse pharmacy family and medical contacts it's a good idea to click the save button as you move through each section especially if you need more time to enter your information after clicking save you'll be able to continue right where you left off continue with family and medical contacts if applicable all fields are optional check with your provider about which information he or she would like to receive from you once you're finished the information provided will be sent to your provider for your electronic chart also located in the profile section is your patient history starting with history of present illness you or your authorized portal owner can submit information about your primary reason for the visit as well as current symptoms if needed click the save for later button to save your progress click continue to proceed through each section in past psychiatric history you can note any prior outpatient or inpatient treatment psychiatric medications substances used and emotional or behavioral problems in medical history you will note current medications and known allergies and answer questions about past medical conditions in social history you'll answer questions about childhood family experiences living situation and social support system financial situation employment and legal history military history sexual orientation and current habits as well as relationship history in developmental history you will provide information from your childhood from birth to developmental milestones last in family history you can provide data about the medical history for immediate and other family members when finished click submit to electronically send the patient data to your provider another feature of the patient portal is the ability to see your patient history as well as an access log noting activity in your portal for both you and any authorized portal owners click into the my history section to view your full chart you can also download a compressed folder containing the xml document of your history if you intend to make this available to another provider click on access log to see a report of each time you have accessed your portal account this concludes the patient portal training series part 2. please continue to part 3.
