It asks about your age, height, weight, height in centimeters, weight in kilograms, date of birth, address, phone number, email address and the last four digits of your credit card number. It also has instructions to "add" your signature so that the form will be printed on paper, and send it off to your local post office. I'm not an attorney, but I do have some friends who have a lawyer, they can give an interpretation/referal for what information needs to be entered on there. I'll pass it on.
