If you wish to attach a printed booklet, complete and file Form 6251, Electronic Notice of Intent to Register the Vehicle, with your application. Note that a printed booklet will be mailed or delivered to the address provided on the back of the application. Do not place the printed booklet in the signature line at the time the application is mailed.
