What would a typical day be in my life as an anesthesiologist?
There are no typical days, so I will describe a sample of what my days are like:Today (post-call):I had the day off. In my practice the post-call people get the lightest schedule. Today that meant no cases for me. I drove my daughter to school, caught up on errands, and ran 6 miles in my neighborhood. However, some days that I'm "off" I may have to pick up add-ons or emergencies.Call Day: Yesterday I was on call. The call person gets the longest scheduled day, the backup call person gets the next longest day, and so on. My day really starts the evening prior, as I call each of my patients to pre-op them for the next day. Half the time the patients don't answer the phone or it's a wrong number, but I always call the night before so there aren't any surprises.When I call patients they have no idea who I am. So when my number pops up on their phone, I have to quickly introduce myself and explain why I'm calling. I gather a very focused history, without any prior information. It depends on the patient, their level of education, and insight into their own medical problems; but sometimes the history is not helpful. Or they leave out major details. But it's better than not calling. A typical surgery starts around 0730. I usually arrive around 0700, depending on the case complexity. This leaves me about 5-10 minutes to review the patient's chart while I'm talking to them. I purposely appear to not be rushed, and to give the patient every opportunity to ask questions. On any given day I may be doing 3-4 cases at one hospital, and may be done by 3 pm, even on a call day, or I may do one or two cases at each hospital, and cover 4 hospitals in a day. In DFW most anesthesiologists are not employed by the hospital. Instead, we have medical privileges at each hospital, which means we are allowed to practice at that hospital and do a defined set of medical procedures and interventions. In any one OR, each room is covered by an anesthesiologist from a different group, with no direct relationship to each other. Often we "follow" an individual surgeon as they travel from hospital to hospital doing cases. It's not unheard of to do one case at one hospital, travel to the next hospital to do a few cases, then come right back to the first hospital for an afternoon case. I cover mainly vascular and orthopedics, with a few other specialties making up the remainder. Covering vascular means that I can get called in the middle of the night for a ruptured aneurysm or an ischemic limb. One time I had the day off, was getting an oil change and was told to rush to a hospital for a ruptured AAA. I told the garage to finish quickly, gave them a wad of cash, and sped out of there. Much of the orthopedics I do is sports medicine, but it's not all healthy patients. Often we cover hip fractures, which are typically frail elderly patients with multiple medical problems. The surgeons post them for 5 pm or later, after they've been "cleared" by medicine, made NPO (nothing by mouth), and clinic is over. So a typical call evening is doing vascular add-ons and emergencies, orthopedic trauma, or all the cases that got delayed from earlier in the day. In my group the call person is expected to break out any non-call people if their day runs late. About those hospitals: I'm in a group practice of 8 people, and we cover 22 hospitals and surgery centers! The furthest South and furthest North hospital are 40 miles apart, meaning I drive a lot.Regular Day:Regular days can be as simple as two one-hour cases in the morning, or a full day with lots of cases and driving. The schedule I receive the prior evening is merely a template, as cases cancel, patients show up late, or any number of things happen. This time of year gets very busy: patients have already met their deductible, so they're all trying to squeeze in their surgery before the end of the year. That all means that even non-call people can work til 7 or 8 in the evening.I hope that answers your question. It was baptism by fire.When I convened a meeting of all the officers connected with the elections to devise a strategy, we found to our dismay that none had any previous election experience whatsoever! So there I was, fresh from JNU & Mussoorie; at the seashore of life.Every challenge contains within itself an opportunity and we set to work de novo. Those were the days of the ballot box and printed ballot papers. Polling parties, members of which are strangers to each other, had to be formed, trained, motivated and supplied with numerous items before being despatched in Trucks to polling stations, many of which are located in tough terrain and inaccessible areas.Our inquiry revealed that all across the State, the formation and despatch of polling parties took two to four days. We asked ourselves can it be done in a day? Can the departure of polling parties be engineered to accelerate this critical process?Research revealed that delay emanated from the fact that there prevailed a spartan boot camp mentality about the whole thing, and polling personnel were not made to feel at ease. On the Sunday morning of despatch I put the time of arrival at 8/30AM, half an hour before "Ramayan", whereas everywhere else it was 11AM, an hour after. My colleagues were aghast, each thinking of his own Ramayan.I said the solution is simple: we will put a dozen TV sets around so we can all begin on an auspicious note, what with on payment breakfast kiosks and tea on the winter morning. Whoa! That simple? Those days, superior bureaucrats were the stern type and this openness was a novelty. And it worked.Research further revealed that counters were opened item wise so that each team had to visit each counter making it a 15 motion affair. If we made it into a one motion affair we could cut down at least one full day. I said we will do it booth wise. With booth nos 1-50 getting all their items from counter A and so on. Simple? All innovation is.Whew! Our guys said sir jaldi karo so we can leave by 2PM, and have lunch on the way and signNow our destination by 8PM. Making them willing participants we did the despatch in a single day. We did the general election 1989 and the state election in 1990 within the same winter, innovating as we went along. Mind you , the sitting CM was from our Sub-Division, and I was his returning officer.I applied my methods in my subsequent postings as SDM Talcher, ADM Cuttack (undivided), DM Sundargarh and Mayurbhanj, and Divisional Commissioner Central Division. In Mayurbhanj, a massive tribal District with four subdivisions spread over 10,000sq kms, we did in a day what it took full four days on previous elections and the skeptics are shaking their heads till this day! I was election observer Raigarh, Thrissur, Jalandhar, & Howrah; and in every election I have left my unique imprint, and my successors have followed. If some of them did not acknowledge their debt to me, well it is all in the game! For me it is #IndiaFirst!

As a police officer, what is the kindest thing a suspect ever said to you?
Over the years, there have been several people (suspects, offenders, witnesses, victims, etc.) who have said kind things to me. One that comes to mind is probably the most recent. As a captain, I’m not often out on patrol and I make very few traffic stops. A few weeks ago however, I saw a vehicle traveling the wrong direction in a traffic circle. Given the danger presented, I stopped the motorist and saw that the vehicle was driven by a tiny African American lady of about 65 (I am a not so tiny 55-year-old Caucasian man), and that she was confused and upset by the unfamiliar traffic control device into which she had driven. I helped get her turned around and pulled to the side of the road where we were both safe. Pursuant to Department policy, I asked her for her license, registration and proof of insurance. She paused a moment and then started looking through her purse, center console and glove compartment for the requested documentation. Based on my experience, this suggested to me that she did not have one or more of the documents I had asked her to produce. I asked her if she had a driver’s license and she admitted that her license was suspended. I couldn’t help but notice the tears welling up in her eyes. I had already noticed that her license plate was expired, as was her motor vehicle inspection sticker. I then asked if her automobile was insured, and she told me it was not.In my jurisdiction, unless there are exigent circumstances, a vehicle found to be uninsured is to be towed and impounded. In addition, a motorist found to be driving under suspension may, at the discretion of the officer, be arrested, and should not be permitted to drive away from the traffic stop absent exigent circumstances which mitigate against preventing the motorist from driving any further. The lady had tears streaming down her cheeks, knowing she had a problem. She was wearing a business outfit, so I asked her where she was going, thinking that I might be able to drop her off if time was a problem. She explained that the previous week, she was hired by a local hospital and that day was to be her first at her new job. She explained that she had lost her home and job in August of 2016 when our area suffered through the worst flood to affect the area in modern history. Approximately 80% of the homes and businesses in our area flooded during the event. Many had never flooded before, were not in flood zones and consequently, many families and businesses were left with nothing, following the inundation. This lady was one of those.She understood that she was in trouble, and she understood that despite her good intentions, she was breaking the law. But there was no anger or bitterness. In her face, I saw only resolve and determination. She never once suggested that she deserved a break. She didn’t hint at leniency. She expressed no self-pity and did not seek to shift the blame. While I may not have shown it, I was very impressed with the way she was handling the experience.I told the lady, whom I’ll call Ms. Smith, to have a seat in her car, turn on her air conditioner (it was about 95), and that I would be back with her as quickly as possible. I then returned to my unit and checked her registration and driver’s license. I determined that her license was suspended due to her inability to honor a payment plan with the DMV that she had signNowed in order to resolve the fines assessed to her when her insurance lapsed. She was uninsured; her history showed that over the last several years, her insurance lapsed several times, She always managed to have her insurance reinstated but the administrative fees had built up over time. Her registration had expired two months earlier. However, with a suspended driver’s license, she could not renew her registration or obtain an updated motor vehicle inspection sticker. Clearly, she was in a vicious spiral and putting her in jail or doing anything to cost her the new job would not be helpful.I carefully considered my options, recognizing that my obligation to protect and serve includes keeping people without the mandatory liability insurance off of the road. Her failure to have the required insurance presented a problem for me. Under our law, were she to have an at-fault accident and it was discovered that I had stopped her and failed to take the appropriate action, I could be held responsible. If I towed her car, she would likely lose her new job which was going to pay her enough to get the required insurance, take care of her suspended driver’s license and generally make her life better for the first time in two years. As I sat there considering what to do, I suddenly remembered something. I got out of my unit, walked up to car and looked in the back seat. There, behind the front passenger seat was a rear-facing baby seat, buckled snuggly into place. “I meant to ask you about the baby seat,” I told her. “Oh,” she replied. “That’s for my grand baby,” she told me. “I take her to day care each morning so that my daughter can make it to nursing school on time. This is our only car.” “Do they live with you all the time.” I asked. “Yes, sir! They’re my best friends,” she said with a smile.I thanked her and returned to my unit. I smiled too and thought to myself, “Exigent circumstances! Can’t tow that car if there are exigent circumstances.” I prepared the lady a citation for driving under suspension. I hated to do it, but the only discretion we have in such cases is jail or a misdemeanor summons. I did, however, exercise my discretion and prepared a warning for the expired driver’s license and expired vehicle inspection sticker. I then made a quick phone call before walking back to her car, ticket book in hand. As the little lady waited to hear her fate, I began. “OK, Ms. Smith, here is where we are,” I told her. “I have to issue you a citation for driving under suspension. I have no choice. However, you’re not being arrested, only fined.” She asked me how much the fine would be and I told her that I did not know, but that I had called the Clerk of Court, asked that they let her pay over a period of six months, and that they had agreed. I then handed her the citation and asked her to sign where indicated. She signed and then remembered the other charges. “Is there another ticket,” she asked? “No Ma’am,” I told her. “Apparently, I am out of tickets and don’t have time to go and get some more.” She seemed confused and asked, “I know you have to tow my car, Captain. I was just wondering if there is any way that you can let me get to work first. I can park it around back where nobody will see it and you can tow it from there, if it’s not too much trouble.”“Well, Ms Smith, it looks like your car isn’t going to be towed today. As it turns out, there are exigent circumstances that allow me to let you keep your car. But you have to get insurance within 30 days.” She didn’t understand, so I explained that since there is an infant in their house, since they had only one car, and since that car was the child’s sole means of transportation to day care, such a situation created exigent circumstances which allowed me to decline to tow the car, provided that I had a reasonable belief that the required insurance would be obtained within 30 days. She assured me that they would get the insurance they needed.I wished her good luck and told her to be safe and I turned around to walk back to my unit as the tears started rolling down her cheeks again. Before I could walk away, however, Ms. Smith called out to me, “Captain Sharp!” I turned back to her. “I don’t know if you’re allowed or not, but would it be OK if I gave you a hug?” I told her I would love a hug, and it was the best hug of the day.I’m sorry that it took so long to answer your question, but, “Would it be OK if I gave you a hug?” was one of the kindest things that a suspect ever said to me. After the Anglo-British war of 1865, the British appropriated the lands that are today known as Kalimpong and Dooars. Therefore, all the people of Nepali, Sikkimese and Bhutanese origin, who were living in these tracts automatically came under the British and subsequently under India (after the British left), hence the term – 'came with the land.'Q. Were there Gorkhas in Darjeeling region prior to the British coming to India?A.Recorded history shows that the region was inhabited as early as the 9th century. When Guru Padmasambhava had passed through this region in the 9th century, he had established Buddhism in the region – which indicates the presence of people living in the area way, before the British ever landed in Asia.In the Indian context, the word 'Gorkha' is an umbrella term used to identify a varied group of people, as one unified entity, in terms of Darjeeling, communities such as the Róng – Lepchas, the Tsong – Limbus, the Kirat – Rai, the Dukpas and the Magars are the aboriginal/ethnic/native people of the region, who constitute a large chunk of the 'Gorkha' people living in the Darjeeling region. Hence, it can be safely said that the majority of the 'Gorkhas,' who belong to these communities and are living in Darjeeling, ethnic to the region.In addition, other groups of people such as the Gurungs, Thapas, Chettris, Newars, Sunwars, Bahuns, Kamis, Damais, Sarkis, Bhutias, Thamis etc., came to the region following subsequent wars. For instance, the establishment of the Kingdom of Sikkim in 1642 brought in a large Bhutia population from Tibet and Bhutan into the region. Similarly, the Nepali incursions starting from as early as 1700s brought many present day Nepalis to the region. Hence, it can be safely concurred that the 'Gorkha' presence far supersedes the British arrival in the region.Q. What historical claims does West Bengal have over the Darjeeling-Dooars region?A.Ironically None! There is no shared history between the Darjeeling-Dooars region, and the rest of West Bengal till the year 1935. The only common thread that connects Darjeeling and the rest of Bengal prior to 1935 is that they shared a common Governor (for administrative ease).It is interesting to note that following the partition of Bengal in 1905, the administration of Darjeeling was handed over to the Bhagalpur Division in Bihar Presidency between 1905-1907, following which a demand for a separate administrative unit for the Darjeeling-Dooars region (a la Separate State in today's term) was first raised by the Hillmen's Association in 1907, making the demand for a separate state constituting the hills, Terai and Dooars region the oldest demand for statehood in India.The only reason as to why such a demand was not entertained by the British Government, is because the Darjeeling and Dooars region were already declared a "non-regulated area", which meant that the rules and laws developed for the rest of India would not be automatically applicable to the region.Q. Why is Darjeeling a part of West Bengal?A.The Darjeeling region only became a part of West Bengal Presidency in the year 1935, when it was required to send an elected member to the Bengal Legislative Assembly. It was done purely for the then administrative ease, as the British could control the Darjeeling region better from Bengal than from Bhagalpur in Bihar.Q. Why are people in Darjeeling demanding Gorkhaland?A.The demand for a separate administrative unit (separate state in today's term) for the Darjeeling region had started as early as 1907. However, the influx of Bangladeshi refugees starting in 1965 and later state sponsored illegal immigrants from Bangladesh post-1971 for vote bank by subsequent West Bengal governments led to marginalization of the ethnic Gorkha, Kamtapuri and the Rajbanshi communities of the region. The demand for Gorkhaland is a demand to protect the identity, culture, history, traditions and the rich bond of people from Darjeeling region, which they share with their land.Furthermore, the Gorkhas from the Darjeeling region have continued to be labeled by the fascist and state sponsored Bengali organizations such as Bangla O Bangla Bhasa Bachao Samity, Amra Bangali, Jan Jagaran Morcha, Jan Chetna Morcha as illegal immigrants and the demand for Gorkhaland illegal. They have rendered the ethnic Gorkha people as an intruder in his/her own ancestral lands. This has caused widespread socio-economic and political marginalization of the Gorkhas. All these factors have resulted in the Gorkhas being under-represented, stereotyped and communally discriminated in almost all sectors.Moreover, Bengal has always been colonial in its approach to this region. The large revenues collected from Darjeeling region have been used to develop other parts of Bengal, while neglecting even the basic infrastructure in the region.Case in point: the National Highway 55, which used to be the artery connecting the hills of Darjeeling to the rest of India, has been closed due to land slide since 2009 and the West Bengal government has done nothing to rebuild it. Last year alone, there were over 20 malnutrition related death (death due to starvation) reported from the Dooars region and yet the West Bengal government did nothing to alleviate the sufferings of the people in the region.Q. Is the proposed Gorkhaland region economically viable?A.The proposed Gorkhaland region is rich in bio-diversity, scenic views, hydro potentials, tourism, NTFP, Tea and numerous other resources. Darjeeling Tea has been accorded the Geographical Indicator status recently by the WTO, which has caused the price of Darjeeling Tea to double in the world market. The demand for tea from the Dooars region is also at an all time high. Even the most conservative estimates put the revenue potential from tea, tourism and hydro from the proposed Gorkhaland area at over 1600 Crores per annum. It is estimated that the revenues from Tea and Tourism alone will make the proposed Gorkhaland region a revenue surplus state. The revenues collected from hydro development, NTFP, cross-border trades and other resources will make the proposed state of Gorkhaland as one of the most economically vibrant states in India.Q. Is the demand for Gorkhaland illegal?A. No, to demand for a separate state is the democratic right of every Indian citizen, and the formation of new states is enshrined in our constitution.Article 3 of the Indian constitution specifically deals with the issues regarding the formation of new states:Article 3. Formation of new States and alteration of areas, boundaries or names of existing States—Parliament may by law—(a) form a new State by separation of territory from any State or by uniting two or more States or parts of States or by uniting any territory to a part of any State;(b) increase the area of any State;(c) diminish the area of any State;(d) alter the boundaries of any State;(e) alter the name of any State:Hence, the demand for Gorkhaland is a legitimate demand.Q. Why is West Bengal opposed to the formation of Gorkhaland?A. The state of West Bengal is one of the most economically backward states in India. Even though, it is the 5th largest in terms of its size, but due to the high debt burden of over Rs. 2.5 lakh Crores, West Bengal is practically bankrupt and is highly dependent on the Central Government and the revenues generated from the proposed Gorkhaland region for its sustenance and economic survival. A conservative estimate indicated that if the proposed Gorkhaland state is formed, West Bengal is expected to lose over 40% of its revenue stream.So despite all the rhetoric stating, "Darjeeling is Bengal's Abhinno Aanga," Bengal is scared of loosing its hen, which is currently laying the golden eggs. It is scared of losing the cash cow that has continued to discount the development of Bengal's other regions over and over since independence.Further, Bengal has always held a parochial, colonial and discriminatory attitude towards the proposed Gorkhaland region and continues to do so. Late. Subash Chakraborty a Minister in the CPI(M) Government had said to the Gorkhas "khetey diyechi, sutey chai?" [Meaning: "we have given you food to eat, now you want a space to sleep?"] Hinting that the Bengalis have been benevolent towards the Gorkhas in allowing the Gorkhas to live in Bengal, while ungraciously ignoring the fact that the Gorkhas are ethnic to the land. Similarly, many Bengali scholars [such as Sumanta Sen of The Telegraph] and politicians [such as Dr. Mukund Majumdar and Mr. Ashok Bhattacharjee] continue to call the ethnic Gorkhas foreigners and intruders, which gives a sense of insecurity amongst the ethnic Gorkhas – thus, the demand for Gorkhaland.Compiled by Upendra Mani Pradhan, Meri News

Sharma had gone to Dhimanpura on Tuesday night for covering the derailment of a goods train, when the railway police personnel cornered him. Before he could reason with them, Sharma was mercilessly thrashed, dragged to a police chowki.Made to drink Urine? Human urine?
