okay everybody we have a really small I guess we call it it's my treat today very cozy the weather in Anchorage is atrocious or getting that way it's deteriorating as we speak I don't know what these group right so I asked her to come here today because I'm constantly any questions from volunteers as to how do I get the service set up who do I call to do this what does it care coordinator to so we thought we would bring up the expert to explain exactly what and you would go on the care coordination director all Tigers resource of Alaska or care coordination resource of Alaska it's very amusing I'm going to give a little bit of explanation on that we when we became a care coordination agency we provide services to all disability and all agents and people that were transferring from their previous agency over to us for like Alzheimer's went away well and with all time so we we created care coordination resources my last day there seems to be a lot of and we have opposite opposite so they just didn't shoot during the same though they want the ADRC out there and there's quite a few other service and then we we don't have anybody going to we want that education we understand the okay so I'm hoping that when you leave here today you're going to have some idea what care forget I'm also going to talk a little bit with services Medicare for you care for a turkey importantly and and then with the available funding sources are to help pay for these services there are a few and not everybody always [Music] about it is today so but but one thing to keep in mind care coordinators are all different I was just saying everybody has a different way of going about it and everybody makes their way is the right way but but there's a lot of different incarnations so you can't find the person that meets the need I've noticed the perforators tend to have sort of a specialty area like some seem to be really great and that wouldn't care coordinator that is great effort through the guys and I've got one that is really focused on medical issue I'm really understand the medical system and then everything yes what kind of to be eligible for a care coordinator we'll see if I remember halation you ideally you have a bachelor's degree and Social Work services psychology so kind of that type of thing and one year of experience of working in the field it's it's very broad or three years of experience or and that also is very broad so I've seen people that and this is not bad if you provided respite for years so there's there are some parameters but you do have to see your disability services training that's out I think if it's ten hours but it feels and then you have to complete once you get certified and you have to complete 16 hours of continuing education every year and every two years - the SDS training and get recertified every two years so I was just like the various agencies also have their own minimum requirements in order to hire care coordinator would they be different than the state ones so yeah what I have found because I it's there is no program that says or degree but says hey this is your care for each two degrees so you really have to read the person's resume and I get what their strengths are I mean I've got what I look for when I'm tired you care coordinators I want to see that they have some knowledge what it is I want to see that they I want to see what their ability to develop bullets are and and then also the one thing about care coordination you're out there talking and a lot of things about care coordination somebody calls you as I have this problem and you're like I've never heard of that before and and so me the telephone in telephone book generation I like to get on I have a huge list of contacts and I'll say who might know something about that so you have to be able to ask questions and sometimes you you don't even know what questions to ask you also have to be comfortable directing your care coordinator you have to be aggressive or nothing is going to get done so you really have to be directing people you don't do necessarily to everything but you have to let everybody know and make sure that they've had time so that's trying to what I look for again interpreted they do like to post my contract but but the team it is a team and it takes every member of that team to to make this word this is a little bit you know with the team can look like the care partners I personally should have changed this slide and that person that circle little circle should be the person and then their formal team as all of the people that are getting paid doctor is the pharmacist assisted living my mom the meal provider you know whatever services that they're getting in there and each person has a important role to play in this and we need to make sure that the team is kept in the loop sometimes that communication piece doesn't always happened as easily and then there's the informal team which includes the person's family friend portable team supports their and and we need to get them to be a part the team so the team can be very big some people who are a lot of times when somebody moves into an assisted living home their circle gets smaller but and it's always important to find yet new kids there it was important in that person's life this next slide I seem to spend a lot of time what a care coordinator can do and which it makes me feel bad that we can't do what we can do is we assess the carers that can determine what their what are their strengths that weakness is what it needs what will meet that need and we a lot of resource providing resources giving information what what did a hell digital for what it's available you know follow the unifying they have a question and I spent a lot of time determining letters that they receive from government agencies as there are to understand and sometimes I'm like what is this need we assist the care recipient with financial with applications and financial applications and I say this because there are so many things that we can't do many times like Medicaid eligible I don't know what your finances are I don't know your bank account I don't have access to manage that is not in my in my job description that would be so what I do is I provide the application I inform whoever is going to complete it what needs to be on it what needs to be included with it where it needs to get submitted I will review it to make sure that it looks reasonable because I don't make the decision but I know from my experience what seems to be approved which is never consistent and and sometimes I will sit down with the client and I'll say what's your name what's your top number now they will give me the information process affiliate of the applications marriage knowledge so so when I say elect people are like I don't like people to go why I will I have a way to a Medicaid application via secure email I do offer to - that includes a release of information so that I can talk to the Medicaid people and then kind of find out get some interpretation because it's very it's very amusing plus it's that you work with hours of attorneys they need we really can't do anything without them saying yes do and I know that a lot of you know it's a lot of work can i I know a lot of people they go on that trip right here you know providing that level of care can be 24/7 and in our agency we average actually by the clients for Facebook which is a lot in a 40-hour repeat if I were going to take on the level of care that that sometimes the representative base we're just gonna do everything we'd be able to have one line so and it would be really expensive yeah we're going to do with people [Music] [Music] [Laughter] [Applause] so it's kind of my I just sounds jaded on my part it seems like it always when way or the other the money is an issue who's making the decision is there not what's being spent on who's got control of it and so that's that's all and we can there's Hades Apple there used to be wrecked a tree there which I thought was great and nobody wants to we should point out to you it's not very often it gets tippy so complicated decisions are made and the person is not in living safe and and we can't really do our job and that you know most people don't really like to hear that you know we make sure that we're not just before the carrot and then we provide referrals so we you know like like I said we're always like would be hurt you know what else would ya I'm the knight that's it I'm going to call them tomorrow and let them know that and then I have down here assist with scheduling appointments and I have a question we as care for people's health care this is where we need to have the team brought in because we're kind of overseeing the bigger picture and so to monitor the day to day needs each every seventh year can be very overwhelming and so we generally we it kinda depends on the situation as to whet our involvement in and scheduling health care we only attend depending again upon the plane and leave a attends the acquaintance so that's it's really be kind of very overwhelming the state keeps adding more things the responsibility of care coordination and and so we're it's a it's not a good thing because we're really we're almost to the point and we we've been our workload has been doubled without any additional compensation and so we're really happy to make sure that we're intending for what's in regulation and fit meet the needs of each individual where it's at so so it's been my experience that many of the I mean they have the schedule they know what's going on in the person's life so I feel like if we do that so we work that out so some of the things I mean we talked a little bit about what we don't do we like I say we don't make the decisions it is it's not we make recommendations it is not our place to make decisions it's tougher when there's no decision-maker there we made we try not to but sometimes but we still never go I mean I guess I could tell you my story I was doing a intellectual disabilities and her health care needs became such that we switched her over to what's all the adults with physical and developmental disabilities she had another service called day habilitation which is going out I was recommending that she go to serendipity because it's it's were consistent you know she was having some health issues a sigh you know it just seemed like a good burger they were having trouble to stand up and the Guardian whenever I looked at it she really liked it the bus seat the transportation was going to be an issue but but in the end she said you know I'm not going to change it and I remember going what that's not right this would be the perfect place for her I've known her for two years she's known her all of her life Who am I to think I know it's the best thing for her so you know I anyway we do not force people to service the I have received a lot of telephone calls from daughters and they're like I need a care coordinator because I need someone that's gonna force my mother to accept services I don't do that and so there's that we like I said we don't do finances we do not transport our clients I say that our agency does not provide transportation to the clients that is a separate service is a huge risk for us we may not have the training that's required to transport audience it may not be safe to the plane to put it in our beat-up car there are there is transportation available for most people I say that of knowing that there are care providers out there that do a lot of transportation there have been a few terms with the care coordinators said if I don't take them nobody else I mean it just isn't going to happen so we have done a couple of times they in general do not transport liens and we do not we can't find it it has to be quiet talking about state services and the person has power of attorney and the state has allowed that power to treat it like we have wished sometimes there is a power of attorney the client I don't find some care physician the the person is still able to make their own decisions but they have a power of attorney in caves and and we've got around a little bit with the state about they go but there's a part straight they have and we're like well no not really so it kind of depends we have to say why the power of attorney but we're any plans back where somebody has a diagnosis of dementia and they have side their own occupation to stand fight they don't and I would argue there by Gavin they also can't find a power of attorney yeah the only option is to petition for a guardian together and what does care coordinator - we assess the screen and form so usually the initial will be personated client we're assessing around whatever he is straight and then we're you know determining what might be the service that would be fitting for them potential service providers while we did not make decisions people usually come to a care coordinator because they I can give them a list of the 400 assisted living homes in the state of Alaska and they're going to look at me take they're fired we are hoping that you can give us some directions so I kind of you know what area of town two-story what level of care where would I absolutely never say and I've learned because sometimes that hope that I would never ever that's great place but so we're always you know looking to see what possible services there's three PCAs here's here's here's a list that I got from the Ombudsman office here's some doctors that specialized disease you know that type of thing that's thing that's going to confuse some folks so let me kind of talk about that for a second so if you're on the waiver you're mandated by the state and the care coordinator Alzheimer's resources we'll do care coordination services to regret something on the labor so those are probably the clients are driving up your short turnaround Airport and I actually will get into the different but but so that's what I'm saying that we refer and inform that's what I'm talking about so well we work together we also they are the major sources any grants that are available Medicaid and I say Medicaid or regular Medicaid or general Medicaid over versus waiting on Medicaid and we again assist with the application so it could be grant application that could be a Medicaid application the care coordinator definitely has to do the waiver application and some people we made to a general lead if yeah I'm hoping okay next slide if the person is going to give long-term care coordination either through the waiver or grant you will develop a plan of care and the Medicare identifies the needs and the desires of the care recipient and and then it identifies what services would be most beneficial and agreed to by the care recipient and then we in this plan we identify whose didn't provide that service and it usually includes the amount of services can be provided and that a look anymore is dictate there's a formula that says service and then then the care coordinator monitors those service those services so we do monthly contacts with that client that came waiver client we have to have contact with either we have to see them one of them we have to have eyes on them and then we have to do another contact and that can be a telephone call do some emailing and we give me a tour with the client or their people representative because sometimes the plant we like to check in with the legal representative and find out they may have concerns that were unaware of and make sure that they're understand so it's kind of like a big cycle eye or a circle I guess I shouldn't say as we're always assessing we're always monitoring we're always you know so we're always worried it just keeps going over and over every year we have to do so it's it's always Raleigh's on top of it to see to eat and more often a first-edition right yeah we we for the Medicaid Waiver we will have there's amendments that the service provider changes or the client needs additional services this is kind of an example this is one section of the plan of care so that's you know we identify witnes services he was going to provide it how much is going to be provided for it's based on their personal health information so you know there are certain entities like I don't know you go in and say I want to see we we do technically we need resident consent to do that but we do have it depends a theory of the home for the care plans on file they can just verbally tell them as okay so one thing that I really noticed in a lot of the senior service assisted living homes I don't know that the staff there knows yeah and it's you know we can use it a lot with the administrator and and when I go in and talk to the staff they they have good knowledge of that lion I was trained that the plan of care is like the bathroom what does this person who they are you know what where their needs where what to do you know their goals and yeah so the next when I listed here where services that are available through the adults living independently Medicaid Waiver except right into care partition that's a mandatory service and who are on a waiver everyone here the residents that are on the waiver II will be he made a lot of waivers yes yeah each piece service has a relatively large list some of your smaller list and others but so these are the things that people could choose from the waiver the thing is is there is there's combinations that you can you can't have like if you live in an assisted living can I get under the videos and I get the scanner chore so if you live in oh no you may be eligible for orbit not respite so there's these combinations of how the services work together the handout that I have has taken out of the care coordination family on and I really like this I use this all the time when I'm meeting with people that are interested it talks about that list out here the different waiver is what their it gives a very free description of their level and care to be eligible for a waiver and care coordinator a half half Medicaid and they have to care coordinator how to submit an application to senior disability services senior disability services will do an assessment to determine if that person needs nursing facility level care which means they have to require significant physical assistance with their activities of daily living is it like three or more Sarah there is a number there's different ways to get there and they it's kind of weird it's an interesting process but and then once they need level and cared then that you have to be the Medicaid eligibility which I talked about well I talked about eligibility will get there to be eligible there so we do a senior in the home there's a senior in will branch out there and neighboring there's different agencies which I also go and step to be eligible to senior in Hope grant six year over load there's also the a TRD grant all Tigers did you and related to punch at any grant there's no income eligibility on that it provides $2,500 for goods and services for people with the diagnosis and now so you can be able to living home what kind of things so we I mean it's really pretty much so it can't I mean it needs there certain things that it won't pay for some electronics one of the things that seems too big request is a reminder Lynch air we've also paid for adult day services for transportation services some people claim the furniture some of those what would be covered by Medicaid so what is supplement like yeah it could be a supplement late late if somebody doesn't have waiver right then yeah they don't get anything and some people may need to go we wanted to go to I know there's one gentleman excites me there's one gentleman that goes to a boxy so then they have another person they have to have a diagnosis of dementia and Kevin silver Alzheimer's research the last guy that I work for manages the grant for the trust Kevin silver at our agency it's called the adrd mini grant so I talked about oh yeah so there's diagnosis for grant nursing facility level of care that means that you need significant assistance with physical ability they look at cognition like I let people with dementia are physically capable they'd be able to follow through on it so like I get a lot of people that assessment is like put your hands over your head put your hands behind your back stand up sit down well most this is very general but alive people so those people used to get denied and now they are looking very closely and so so there's look there's it's not matter and then for people that are very Atrus routine care for that we can also there is personal care services that you hope they don't get that in a system that is very definitely based on physical abilities and you have to have that case so funding sources so number one private pay we were just talking about the posture assistive living I put on here what I know to be like assisted living homes rage I think I seen one for $4,500 a month which I was really surprised at it seems like the average six thousand maybe six to seven thousand since the average probably have better I call these people we get we do get people that what I see to be an average for like personal care respite you know whatever you want to call it it's anywhere from 25 and then as we were talking our agency will do private pay care coordination I'll get more into the grants services which can which is the next slide so as I was saying before there's senior in who grants that are going to try to state of Alaska you have to be over 60 and have a look at it which means that's kind of a I think because it changes every year and I think right now holy pet foods less than the enemy I always have to look at that but it means less than $2,000 a month or less than 23:13 and then they look they go they change it all the time so I'm never quite sure or low income in an ad Rd diagnosis so there's a lot of people that are under 68 that has some type of dementia and they would be eligible for this program as well and within that brand you can have access to care coordination adult day services through serendipity just in a fridge so does care coordination adult day through serendipity in Anchorage all of these are anchorage every area has some kind of funding the more populated areas you have more choices so like south southeast has their own program at different agencies but but these are the kinds of basic things that you can get chore and respite I'm using those as kind of placeholders there's also some kind of PCA services that they provide under this and in acreage compass homecare is the contact actually compass Co is part of some other agency from the southeast and I can ever remember with their name as they have to say no sale no presenter for community maybe build something else yeah and she said those language yeah coming based on the building see I can't remember maybe Center I can't then there's the mini-grants these are through the trust there's going for Alzheimer's resource that should be Alzheimer's disease and related disorders there's a ramp for people with oh and then for as I said these grants are all maximum of $2,500 purpose oh dear there is an application that's a pretty straightforward application I think with all of them you have to demonstrate for a diagnosis that allows that so they need there so just the grant flew to the first of her to the provider so the grant goes to the provider they they do not reimburse and it kind of depends like we rarely had a chat with the person I mean if it may happen a couple of times depending upon what that means they usually like I know last year this family needed some respite and and they wanted a family member that lived out of state to do the respite and so we got frantic route to pay for the airfare I didn't pay for and so what they did was they sent the family member didn't employees attach the cost of the tickets but an invoice for the word sent it to Kevin and he had the check to the family rather than giving this family you're paying the bill so okay regular Medicaid there are many ways to get medicaid eligibility and there are many different income limits that give you Medicaid I'm just learning so yeah so I try not to get too involved in that but if you are eligible for regular Medicaid at the I think that an income limit is like 13 I was talking to somebody yesterday I was well it's either 1352 or 1369 or 13 and I've actually called them and I said what's the income limit for radio and Medicaid and nobody can tell me they're like we'll just submit an application and we'll let you know whether you're eligible because it's the same application that could get you this sleepy or regular Medicaid a different application so it's really that's why I say what regular Medicaid will cover depending on the flavor what I call regular Medicaid has said that they're seeing something lutely in its medical insurance it is und can a lot of people want it because they get personal care services and I say it's based on the household income so they're going to look at if there's five people living in the house and everything which is different than waiting for Medicaid and not the next slide I have General relief next to anybody can fill up that application that does not have to be do you do it it's there are income limits which I believe are 23:13 per month no more than two thousand dollars of savings when house one card I think you can have one house there's a current waiting list for those services I was talking to a social worker a pack this morning she said that they've been noticing it's take me up to two months which i think is good to get clothes off in the waning months and it is only for long-term care Medicaid this is waiver Medicaid income limit two thousand three hundred thirteen dollars per month or less one home can't be more than one arm this is based off of the individual offer the care recipient not their spouse they will collect the information for the spouse but but the spouse can make whatever they make there only a look at that individual they the individual can't have more than two thousand dollars in cash assets the spouse though they look look there are some limits with the spouse the spouse can't have more than 2600 twenty-six hundred thousand dollars right around there and it fluctuates a little bit so like the spouse doesn't have to be impoverished for their loved one to get services it depends it depends and that's where those it depends it's like I give this is the basic information the lighting application that apply put everything on there and what GPA Department of Public Assistance will be in the application and you are either going to be eligible or they're going to tell you why you're knowledgeable and then you would address those issues so I don't try to tell I have a lot of people to go am I eligible and I'm like let's fill out an application and find out and you know the process isn't as long as it used to be and at least it's pretty quick if you're not eligible and there's no penalty on how many times and actually sometimes you have to apply every two months just because they GPA is time one rate is different than SDS is timeline and and to get everybody to be happy you have to keep applying for Medicaid so when you say LTC Medicaid this is people in the whole system long term care Medicaid is for adults for nursing home or Medicaid Waiver boo-boos yes and totally the same level of care and that's very confusing so tippy you can this is only based on your audience right yeah they don't look there's a lot of confusion I just to somebody to talk to somebody at TPA they are looking at is money and this guy at DK says oh I can get you approved for the waiver and the guys calling me gone he's gonna get me approved from the waiver and I said no he's gone you can financially qualified but we talked about this you did not need level of care so you have to be for long-term care Medicaid get to be skilled care for to actually get the services yes so so this is just the first step in the financial process then there's a whole other application process you're carrying yeah and if you don't I put a slide in here for T care and I'm with this this is you know with do you know I think I was directed by analyze but I have got only greatness that because it could be relevant here this is for it's called tailor tailored care and referral or something but this is a project that is going to be started it's a Grantham senior disability services got and there we're going to be working with them to do assessment of its work dementia clients that are not eligible for other programs and we are doing an assessment to develop referral referral sources and and there's even a small amount of funding that goes along with this pitch and spin we're still in the waiting process I'm hoping that it starts in January I'm just kind of playing people know that this is a potential for people that do not care maybe people that don't want to be in assisting it could provide some resources in their own home I don't know if somebody that we're living in an assisted living home under general relief and needed care needed additional services to that's a question that I need to ask and I will let you know if that body in general relief situation so actually this is an assessment is based off of caregivers and the goal of the caregiver yes so it's really looking at those caregivers that are it's trying to delay the placement and so they're really looking at the stress level and the and so the questions I mean it ask questions about the care recipient and the caregivers the level of care the amount of time that's spent on the service that is not based on it and then it's a database program and it spits out these recommendations and so based off of that that you develop a point of care to help ease the burden for the caregiver so I don't know how much you guys will use this but we're just trying to let people know that it's gonna be out there I'm hoping that it's up there maybe in January when we get phone calls from the general public that might be something a Harry could help them with no no we went to the meeting you're in the meeting Gail Gail said that to find out because I was under the impression that maybe they don't really do it anymore that bending is very good I thought we had people in Shepherd I would listen to that yeah I think my first started working here that was a big table first thing to get out of it services I think that that could be use that way the one thing with t cares they cannot be using they can't have that big waiver okay I was asking what I'm not sure is can they have just I mean if that's I need to ask right that would be whatever Thanks let me go tell me we've got somebody right now then other than a side issue that's come up but the GR president suppose do they know do they have a care provider I don't know right now the situation is they're very independent anyone online question still here yep questions great answer to your question on the chat box yes
