Anyone can lie, cheat, or steal. This is the first thing you need to do. The second thing to do is to think about what you want to transfer before you sign. I will not go into all the details for this document, but if you do not really have the time to go through all the problems, you can just skip it. I would not sign this document, in fact. I would probably sign the one-page loan document on the left because that is the simplest form and it just says you will send a letter to the bank, and you don't really have to worry about any of the details.
