like your lower back your upper back yeah I like my lower back and I'm like the center okay so the center does it radiate to new worlds yes when it's wouldn't it cuz it comes and go so when I'm having pain it does spread to the near areas and then how would you describe this back pain from one being to that pain no pain at all to ten being with the worst pain you've had in your life starting to get worse so I would say about it seven okay so it's pretty up there than okay a year that no is there anything that makes your back pain worse I mean you say you were picking out boxes but anything else that makes your back pain or if I'm trying to you know pick up stuff and that's limits were certain depending on my posture also I'll start having back pain again okay what about anything that makes it better I did try to take like a like a an Aleve for the pain okay did you take anything else no I'm just and what was the outcome did you get better or somewhat um I didn't see much of an improvement okay have you tried it oh you said like any other type of you know probably besides the leave that you took but home remedies or anything like that any stretches for your back no I haven't trade stretches but I did do like a nice cold patch okay did that work um some why I mean it felt it felt good but I still good you know are you having any other type of send some symptoms besides your back pain no okay alright so I'm just gonna go over the information just to make sure I have everything correct or pickin if I don't see anything just let me know so you've been happy it's like lower back pain and stuff going on for about five days and then you notice that it when you're in the process of moving and just kind of notice that whenever you pick up certain things okay you said it's about a 7 out of 10 where you're acting so that's pretty up there again and you took some relief but you know it hasn't gone the pain hasn't gone alright okay you also took you tried some ice okay what's up a reaction do you get with unit like my throat starts to close up and I feel like I can't breathe so you kind of do get a severe reactions last time you get this reaction um as a ten-year-old and ever since then I haven't been allowed to eat peanuts okay and you told me besides so over counter medications that you were taking for your back pain are you taking any other type of medication no no okay so just so that leave together okay what about any illnesses that you had in the past okay so you're pretty healthy yeah and what about hospitalizations have you been hospitalized in the past no well I did have a surgery at the hospital had an inguinal hernia her yeah okay when was this mm-hmm I think I was like in second or third grades and what was the outcome that everything yes everything well okay hmm and then you said that surgery you've haven't had any other surgeries no okay what about any traumas me major accidents that you had in the past and I don't have you travel in the past few months yes okay would you go out to Africa did you go to for about a week or something with it like a vacation or yeah I was a vacation that's awesome do you with your family yes pay well that's awesome I really want to go to Africa it's an awesome so now I'm just asking some questions in regard to your reproductive history it's just some questions that we have just so I can you know you're healthy okay so how do you been pregnant before yes okay - it was good - you have a tee children yes okay boy girl or two girls two girls okay that's awesome hopefully once five another one seven okay great and they told me you have had children have you had any hope you want complications or anything okay no no okay so now that I did ask you about your protector of history I'm just going to ask you about your youth the illnesses how do you had all your vaccinations yes okay and what about your flu vaccination you've had that also yeah last year okay good okay a okay look and what about anything anybody else what about any pregnancy related issues exposure whether it's in your job okay thank you for giving me that information now I'm just gonna go over some social history so can you describe to me what your day is like your daily dance like I wake up like around 6:00 in the morning get ready I got the kid at school and then from there I had to work and then I get out for two and no you told me that you you are you busy since you're working and you have to go home and pick up the kingdom how would you say your nutrition is do you get your three meals a day yes um in the morning I make breakfast for myself and the kids and then for lunch I usually pack my lunch if not I'll go out and buy something for dinner and I usually cook out for dinner okay and how often do you go out and eat I try to keep it to a minimum so probably two or three days max for the week yeah really you know since you're really busy it's a lot easier to go and what are your usually your plates consists of or your dinners or lunches breakfasts I usually try to do like you know a portion of like protein and I like vegetables asides and that's pretty much it okay so you do have that's great you do have you know everything equal you get your vegetables that's awesome how many glasses of water do you have a date um I carry around at work so I really maybe like four times throughout the day okay okay um and in regards to exercise do you get your would you say you exercise I tried to sometimes at home I try to I have a treadmill so I get on when I can you got to do you know anywhere from 15 or 30 minutes of running or walking okay that's good in how many days a week do you usually do that about three much of them yeah they're doing those well the good thing is that you at least know working out you know that's great okay so now I'm just gonna ask you um do you smoke no no okay what about training did your Campbell every now and then not too often it is just social drinking yeah okay so how many times a week would you say if you do drink probably like once a week I got some football Sundays when we get together and what do you drink usually like a margarita and how much do you have usually over um anywhere from one to two margaritas these are just social in okay and you told me that you work where do you work at I work out of school okay are you a teacher I'm a secretary Oh in Secretariat okay so you were just a school secretary yes okay great and and in regards to your support system who would you say is your support system my family we're all very close so I definitely rely on them you know to kind of look after my kids or just you know just family and then I have a few friends close friends that I can also rely on okay great so it'd be your family your friends so it looks like you have a strong support system is good no you told me you do have children um are you married no I'm divorced Stewart so hey are you sexually active no no okay okay alright and how many did it just live in your home you I know you told me you have two children and then I would assume yourself is this anybody else live in your household um no just
