After I received the paper version of my first medical record from the NIH, I took a pen and paper to the file and wrote the first two letters of each row, beginning with "Dr. Smith." I copied the entire row, and then folded it back up, placed the paper in a plastic baggie, and put the plastic baggie in front of the file.
