actually two ways you can apply you can apply online or you can complete the forms if you would like to complete the paper forms you can click on the complete the participating party agreement scan code through national motor freight traffic association you will need to complete the standard carrier alpha code application as well but this application will be faxed to the fax number to the National Motor free this does not get sent to our office and if you have any questions on that you can give the National Motor Freight a call their website is nm FTA org but if you click on this link it will take you right to the area where you need to register for scat code now if you already have a Sky code of course you wouldn't need to complete this application the insurance information if you're wanting to do it paper form you would need to complete the form 5 C which is the equipment providers list this is just letting your agent know exactly who it is that you're trying to do business with and it also lets us know who you're trying to do business with so we can get you set up properly now if you're not sure exactly who it is you're trying to do business with you can mark all of them we don't charge any extra fees for a few more call of the provider or if you're only doing business with one or two providers you only want to work off those one or two providers so if you go back to the top and if your company's wanting to do business with equipment providers and you want to apply online you would simply click on the apply online and it's going to bring you to this page this gives you a small outline of exactly what information you're going to need in order to register you're going to simply click continue and I've completed all of the information that I needed to complete on this now if you don't have a scat code already you can leave this blank and then let us know once your scat code is assigned to you by national motor freight and then we will add that scat code for you now you do need to have a scat code a Medicare number and the USDOT number in order to show approved in our system you do not need to have the numbers in order to register so the day that you register if you can't remember your numbers or you don't have them and you're still working with FMCSA for your motor carrier you do t or you're working with national motor freight on your scat code you can simply just leave those blank and then we can go ahead and fill those in for you but again you will need to have those numbers and the scat code in order to become approved in our system the other part here is the application business operation you just need to mark what kind of company you are so if you're for hire a private carrier which is a not for hire company interstate interesting interested is when you only drive in your own state you don't go outside of state and then the commercial zone terminal area operator if you have a company website you want to make sure that you put your website address here if you are a smart way carrier you just want to check that box and then all of your contact details you can put in your first and last name your title your email addresses if you have more than one email address you can put your email addresses here the primary and secondary email will receive the same emails anything from our office will be sent to these two email addresses you're going to put in your address information your phone in fact your number is required so if you don't have a fax number you are required to have a fax you can sign up with eFax it's a computerized fax and it's usually free if you get so many per month but you'd have to check with the fax and it's a fax calm if you have a billing address like a counting Department in your office and you want the accounting department to only receive notices regarding invoicing for your company through the uia you can put your billing contact information here if you want the primary contact person to receive the billing information from the uia office you would simply leave this saved at yes and then all of the invoice information would go to any of the email addresses that we equipment providers list you're going to mark off any of the providers that your company wants to do business with now and also probably in the future just so you don't have to keep coming back to this list if you are not quite sure you can go ahead and mark all of the providers if you do want to mark all the providers there's going to be one provider that is ramped specific and if I go to click Save on this it's not going to let me move to the next page because Union Pacific is a railroad that requires you to mark off the ramp locations that you will be utilizing for them if you're not planning on doing business right now with Union Pacific you can simply uncheck Union Pacific and that box will go away and then you can move forward but if you don't uncheck that box you're not going to be able to move forward to the next page if your company only wants to do business with one or two providers you're only going to mark that one or two providers you're not going to mark every provider on here and then you just simply save it at the bottom this gives you the information on those two providers as I marked off so I marked off APL and CMA so this is for your insurance agent this is a smart checklist and it's just letting you know what CMA and APL requires you to have so it's showing the auto line ability needs to be at 1million it needs to have the limit the type of autos the standard endorsement attached anything in red is what we're looking for that you're going to be required to have in order to do business with these two providers so this smart checklist is mainly for your insurance agent so you can generate the PDF if you'd like and then forward it over to your insurance agent but if you're not sure exactly who it is that you're doing business with and you marked all of them you can just simply scroll down to the bottom of this screen and hit continue now this is the insurance agent code area this is going to be emailed to you and you're going to need to forward this email over to your insurance agent your insurance agent will need your company's scat code and this insurance agent code in order to apply your insurance online so it's very important to forward this email address that you've received for this code over to your insurance agent so that they can apply the insurance for you and you just want to simply click on save and continue now this is your payment information the payment is manual fee we can take checks so if you want to leave it at check you're going to make a copy of the check and fax or email it to us and then put your original check in the mail if you want to pay by credit card you're going to click on the credit card box and you're going to put in your credit card information here if you do not if you want to pay by credit card but you do not have your credit card on you now you can just simply keep it to check and hit save and continue that will allow the system to move forward you can apply your payment once you get done this area here is to agree with the terms and conditions of the agreement so you do need to check off this box if you need a copy of the agreement you can always get a copy of the current agreement off of our website at you iia org if you would like to have the terms and conditions of the agreement now you can click on this link here and it will provide you with a PDF of the agreement you might want to make sure that you print your signature page to the uia this page is going to just come up in a PDF form you want to sign it you're going to sign it here you're
