fee so if you click on this link here that says annual fee schedule these are the only two pages that you need to send the UI is so again you can either fax them or email them if you need a company scan code through national motor freight traffic association you will need to complete the standard carrier alpha code application as well but this application will be faxed to the fax number to the National Motor free this does not get sent to our office and if you have any questions on that you can give the National Motor Freight a call their website is nm FTA org but if you click on this link it will take you right to the area where you need to register for scat code now if you already have a Sky code of course you wouldn't need to complete this application the insurance information if
