Certificate of completion initial
Upon completion of a REEL project, the Certificate of Completion is where you, the contractor, will provide information on that project to certify that the project is complete. Once filled and signed, this form should be sent to the lender. There are several different sections to the Certificate of Completion - we will address each of them in this video. There is a box marked "TO BE COMPLETED BY PFI or PFL" at the top of the Certificate of Completion. Leave this section blank for the lender to fill out. As a Participating Contractor, you are responsible for completing the rest of this form. 'Section 1 - Contractor Information' is where you will enter your Participating Contractor Business Name and CSLB license number. List your name as you provided it on the Participating Contractor application. DBAs are acceptable if they are part of your official CSLB license. You can use the tab key to move through the fillable fields on the PDF, or you can use the cursor to select each box individually. 'Section 2 - Project and Safety Test Information' is where you will enter information specific to this project. Start by entering the project start date, this is the date the installation began. Remember, this date has to be after the date you became enrolled in the program. Next, enter any permit numbers for permits obtained or in process while completing this project. If a permit is required for your project, you must provide the permit number. If no permit was necessary, check the box marked "Not Applicable." Next you will enter the project address, as well as the City and ZIP Code. Note that the project address should match the service address on the customer's utility bill. Now you have to evaluate the project scope to determine if the project included three or more EEEMs, including at least one of the measures described here: Whole building air sealing, Duct sealing and/or duct replacement, or Attic insulation and air sealing. If project you completed does meet this criteria, check the box marked "Yes" here, otherwise mark "No." If you answered "Yes" to question (h) please make sure to fill out the rest of Section 2. The second half of Section 2 is where you will enter information on Combustion Appliance Safety tests or Combustion Appliance Zone tests. You only need to fill out this section if you answered "Yes" to question (h). This example project required a CAZ test. If you need to fill out this section based on your project's scope, select the type of safety test performed (CAS/CAZ or NGAT) and the name of the company that performed the test. This may be your company or a third-party testing firm but we need to know who performed the test. The company may also be a licensed contractor, if so, enter their CSLB license number. If the contractor who performed the safety test is NGAT or BPI certified, please provide their certification number. If the contractor company who performed the safety test is approved to participate in an IOU, REN, or CCA whole-house retrofit program in the IOU territory, please indicate which IOU program using the check boxes provided. In 'Section 3 - Utility and Rebate/Incentive Information' you will provide information about the utility providers for the project property, and any rebate or incentive information relevant to the project. In fields (o) and (p), please indicate which utility company provides the customer with gas and electric utility service. If the utility is not one of the Investor-Owned Utilities named specifically on the form (for example, SMUD or LADWP), please check the box marked "Other" and write in the name of the utility. In our example, the Borrower gets both their gas and electric service from SDG&E. Make sure to enter both the gas and electric providers for the project property. Next we will provide information on whether any rebates or incentives have been, or will be, received from an IOU, CCA, or REN program. Please include the name of the IOU providing the rebate and the name of the program - for example, 'SDG&E home upgrade Program.' If it is available please include the rebate or incentive ID number. Be sure to enter the rebate or incentive amount, and clarify whether that is an actual dollar amount or an anticipated dollar amount. In this case the customer received a rebate and it was not applied directly towards the cost of the project (in other words, the customer will receive a check from the utility company after work is complete). 'Section 4 - Certifications of the Participating Contractor' describes everything that you, the Participating Contractor, are certifying about this project. Please take time to read over and understand each of the certifications then print your name in field (w). Finally, make sure the Certificate of Completion is signed and dated. The signature section may be completed by attaching a digital signature and saving the completed form, or by printing the form, adding a wet signature, and creating a scanned copy of the form. Please make sure the CoC is signed by an Authorized Signatory within your organization. Once you have reviewed the Certificate of Completion to ensure that you, as the contractor, have filled out and signed the form correctly, it is ready to be submitted to the lender. For best quality, you'll submit a PDF to the lender. If you're unable to submit a PDF, you can take a picture of the form. Thank you!
