[Music] hi welcome back to Camp Corey again I'm Asha I'm admissions coordinator and I'm going to walk you through the final process of completing the remaining forms in order to be enrolled once we have received your Camper's medical provider form Clinic notes and code vaccination proof for each individual attending Camp you will receive an enrollment letter from me and our admissions team and you will then need to log back onto campsite to complete the last few steps before coming to camp once you log back into your profile you will click the forms Tab and there you will see the remaining information we need from you first the emergency contact and medication information form asks for you to provide up to two emergency contacts these can be anyone from family members to family friends please share their full name their relationship to your camper as well as their phone number [Music] we then ask for you to provide some information on the medications your camper may be taking please complete each question for every medication your camper takes we ask for the name and strength of the medication the dosage the reason your camper is taking it how this medication is given some examples are orally or through injection you also ask for the start date an estimate of end date for taking this medication as well as any specific notes regarding this medication in your camper and finally when this medication is taken by your camper what time of day if you need to add any more medications to this form you can add a medication by clicking the gray add another medication button at the top we then ask about any medications your camper should not be given a few examples are Tylenol cough drops and Sudafed please select any on this list that our medical staff should not give to your camper while at camp after this form is completed please give your signature and select submit form once that form is submitted the remaining information we need from you is regarding the immunization record of your camper here you will need to upload the date of each dose for every vaccination your camper has received this form carries over every year so you will only need to update it once your camper receives additional doses for vaccinations if anyone under the age of 18 is coming to camp we also ask for you to share their immunization records by emailing those to our admissions team along with proof of code vaccination for each individual attending camp thank you once this form is submitted the only remaining thing for you to complete is the dietary information form which is linked in the enrollment email you are sent if you have any questions about any step of this process please don't hesitate to reach out to me and the admissions team we are happy to help and support you thank you for following along...
