welcome to the remark software YouTube channel in this video we will show you how to create a training evaluation form in Microsoft Word for use of three Markov Asilomar start off in Microsoft Word by creating a table for the date and identifying characteristics for the training session we'll start off by creating a section for the date greet labels for the month day and year leave space for the respondent to write the date followed by a buffer row then enter the bubbles to be filled in using the OMR bubble spot the Oumar bubble spawn will automatically install in your windows font directory with three markov Asilomar once the bubbles are filled in we'll adjust the width of these cells and then move over to the section for the identifying characteristics of the session we will ask respondents to enter the instructors name the title of the training session and the sessions location we will create labels for all three identifiers x' and extend the width of the column so that each label is one line then we will extend the width of the next column as these will become lines for the respondents to write their answers finally we'll remove most borders from the table we will keep a few boarders that will serve as lines for the respondents to write in their responses then create another table for the objective questions label the answer choices using a Likert scale ranging from strongly disagree to strongly agree we will adjust the width of these cells and then move over to the evaluation criteria adjust the width of the first row and then enter what it is you want your respondents to evaluate to create the answer selections will again use the OMR bubbles font filling in the rows with 1 through 5 we'll then increase the height of the rows to add a bit more space finally we'll remove the borders to complete this section the last section will be an open-ended question asking for potential changes create a simple 1 by 2 table in the top cell write the question asking for feedback regarding your training session then extend the width to match the rest of the form then extend the height of the bottom cell as this will become the boxed or the respondent to write his or her response again finish off this section by removing some borders from the table you have now finished creating a training evaluation form that can be used with remarked office omr we do advise you to test your form width remark office Omar before distributing mass quantities thank you for your interest in remark software for more information to download a free demo or to schedule webinar please visit remark software com
