Frederick Strathmann assistant professor of pathology at the University of Utah and medical director and toxicology at ARUP laboratories in the next few minutes I'll address some commonly discussed topics in urine drug testing regarding quantitation confirmation and dose compliance determination using urine drug test results most of you are likely familiar with the screen with reflex to confirmation workflow regarding drug testing this commonly starts with the collected sample analyzed by a screening methods such as an immunoassay and if the results are positive and confirmatory tests follows in a non forensic setting the initial amino acid-based screen often provides little clinical value for example consider the following scenario about a medium risk patient currently prescribed if a physician ordered a standard drug screen with reflex to confirmation for this patient she would expect negative results for assays targeted towards drug classes such as benzodiazepines and amphetamines but would also expect a positive result for an opioid assay the challenge for the physician is how best to interpret that positive opioid result for an opioid immunoassay targeted against it is expected that various other opiates and opioids will cross react to produce a positive result in the case of our example patient compliance with the prescription would be an appropriate interpretation of a positive opioid screen but because our patient is considered medium risk other more concerning possibilities exist such as heroine abuse ingestion or use this positive opioid screen result is a classic example of a largely uninformative result that requires further testing using a more specific method in the clinical toxicology laboratory the more specific method is often chromatography coupled to mass spectrometry in addition the follow up tests conducted by mass spectrometry typically provides a quantitative result such as 327 nanograms removal of instead of a qualitative results such as positive or not detected but it is important to keep the concepts of confirmation and quantitation separate for instance at ARUP we offer several targeted drug screening assays that use liquid chromatography time-of-flight mass spectrometry the results are reported as positive or not detected based upon established cutoff concentrations much like traditional screening assays but instead of using a nonspecific method such as an immunoassay we use a highly specific form of mass spectrometry for many applications and non forensic use a qualitative mass spectrometry-based targeted screen provides positive identification with no need for further testing instead of reporting a single positive for the opioids class our time of flight mass spectrometry targeted screen will report results for individual compounds such as codeine and many other drugs drug classes and their metabolites to further disentangle the concepts of confirmation and quantitation let's take a look at another example here we have two patients each with prescriptions for patient one also has a prescription for while patient 2 has a prescription for a targeted urine drug screen by time of flight mass...
